摘 要:目的 探讨超声引导下粗针活检(CNB)在甲状腺结节诊断中的应用价值。方法 对75例甲状腺结节CNB和手术病理结果进行了比较。结果 75例75个甲状腺结节取材均满意。CNB病理结果提示乳头状癌53例,结节性甲状腺肿13例,滤泡腺瘤4例,慢性淋巴细胞性甲状腺炎1例,不确定病变4例。75例患者中,53例恶性和19例良性结节CNB和术后病理结果一致,假阴性3例,无假阳性。CNB诊断恶性结节的准确率为96%、灵敏度为94.6%、特异度为100%、阳性预测值为100%、阴性预测值为86.4%。CNB术后出现局部出血4例,血管迷走反应1例,经对症处理后恢复。结论 超声引导下CNB是诊断甲状腺结节的一项安全、准确、微创的方法。Objective To explore the value of ultrasonography guided core needle biopsy(CNB) in the diagnosis of thyroid nodules. Methods A total of 75 nodules in 75 patients that underwent CNB and operation were analyzed in this study. The pathological results of CNB and surgery were compared. Results All samples obtained from CNB were adequate for diagnosis. Of 75 thyroid nodules, the samples of CNB were pathological diagnosed as papillary thyroid carcinoma in 53 nodules, nodular goiter in 13 nodules, adenoma in 4 nodules, Hashimoto’s thyroiditis in 1 nodules and undetermined results in 4 nodules. Malignant nodules in 53 patients and benign nodules in 19 patients had concordant CNB and pathological results. Three nodules had false negative results. No false positive results were discovered. The diagnostic accuracy, sensitivity, specificity, positive predictive value, negative predictive value for diagnosis of malignancy were 96%, 94.6%, 100%, 100% and 86.4%, respectively. After CNB, local hematoma in 4 patients and vasovagal reaction in 1 patients occurred. All relieved by conservative therapy. Conclusions Ultrasonography guided CNB is a safe and accurate method in the diagnosis of thyroid nodules.
