摘 要:目的:观察牙周炎大鼠造模处及同侧下颌牙槽骨的骨微结构变化,初步探讨牙周炎局部造模对同侧下颌牙槽骨骨微结构变化的影响。方法:8周龄SD雌性大鼠12只,随机分为EP组和CON组(n=6),使用正畸结扎丝结扎EP组大鼠上颌第一磨牙牙颈部,建立牙周炎模型,CON组为对照组。建模8周牙周探诊探查牙周袋深度变化,观察牙周组织色、形、质改变,造模12周后处死大鼠,使用Micro-CT对大鼠上颌第一磨牙造模处牙槽骨及同侧下颌牙槽骨的骨微结构变化分别进行分析。结果:建模8周后CON组大鼠牙龈正常,无明显炎症;EP组大鼠表现出牙龈炎症,部分大鼠结扎区可探及较深的牙周袋,牙龈红肿,探诊易出血,非结扎区牙龈无明显炎症。12周后EP组的上颌第一磨牙牙槽骨骨密度(BMD)、骨体积分数(BV/TV)和骨小梁厚度(Tb.Th)显著性降低;骨小梁数量(Tb.N)和骨表面积/骨体积(BS/BV)则显著性增高(P<0.05);同侧下颌牙槽骨骨密度(BMD)、骨体积分数(BV/TV)、骨小梁数量(Tb.N)和骨小梁厚度(Tb.Th)显著性降低,骨表面积/骨体积(BS/BV)和骨小梁分离度(Tb.Sp)显著性增高(P<0.05)。结论:Micro-CT作为辅助观察工具,初步观察到了牙周炎模型大鼠造模处牙槽骨及同侧下颌牙槽骨骨微结构的破坏。Objective:To observe the changes of bone microstructure of homolateral mandibular alveolar bone in rats with experimental periodontitis,and to investigate the effect of local periodontitis on the changes of bone microstructure in homolateral mandibular alveolar bone.Methods:Twelve female SD rats aged 8 weeks were randomly divided into EP group and CON group(n=6).Periodontitis model was established by ligating the neck of the first maxillary molar of EP group with orthodontic ligating wire,and CON group was the control group.After 8 weeks of ligation,the depth of periodontal pocket was explored and the color,shape and quality of periodontal tissue were observed.After 12 weeks of ligation,the rats were sacrificed,micro-CT was used to analyze the changes of bone microstructure of alveolar bone at the modeling of maxillary first molar and homolateral mandibular alveolar bone.Results:After 8 weeks of ligation,rats in CON group had normal gingival condition without obvious inflammation.The EP group showed gingival inflammation,and deep periodontal pockets could be detected in some of the ligated area,and the gingival was red and swollen,easy to bleed,while there was no obvious inflammation in the non-ligated area.After 12 weeks,the maxillary alveolar bone mineral density(BMD),bone volume/total volume(BV/TV)and trabecular thickness(Tb.Th)in EP group were significantly decreased.Trabecular number(Tb.N)and bone surface area/bone volume(BS/BV)were significantly increased(P<0.05);The homolateral mandibular alveolar bone mineral density(BMD),bone volume/total volume(BV/TV),trabecular number(Tb.N)and trabecular thickness(Tb.Th)were significantly decreased,while the bone surface area/bone volume(BS/BV)and bone trabecular spacing(Tb.Sp)were significantly increased(P<0.05).Conclusion:Micro-CT was used as an auxiliary observation tool to preliminarily observe the changes of bone microstructure of homolateral mandibular alveolar bone in rats with experimental periodontitis.
