健脾止泻汤联合推拿治疗小儿腹泻

摘　　要：目的研究健脾止泻汤联合推拿对小儿腹泻的治疗效果。方法选择88例腹泻患儿,随机数表法分为观察组与对照组,各44例。对照组予西医对症治疗,观察组在对照组基础上予健脾止泻汤联合推拿治疗,2组均连续治疗3 d。观察2组治疗3 d后的疗效,记录2组临床症状消退时间,比较2组治疗前、治疗3 d后的中医证候积分、免疫学指标及血清炎性因子水平。结果治疗3 d后,观察组总有效率(97.73%,43/44)高于对照组(81.82%,36/44)(P<0.05)。治疗3 d后,2组大便性状、大便次数、腹痛、烦躁萎靡、呕吐、口渴评分及血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平低于治疗前,且观察组低于对照组(P<0.05)。观察组腹痛消退、止泻、恢复进食、纠正脱水时间短于对照组(P<0.05)。治疗3 d后,2组外周血CD4^(+)/CD8^(+)、血清免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、白细胞介素-10(IL-10)水平高于治疗前,且观察组高于对照组(P<0.05)。结论健脾止泻汤联合推拿可提高腹泻患儿外周血CD4^(+)/CD8^(+)、血清IgA、IgG水平,调整血清炎性因子水平,改善机体免疫功能,促进临床症状消退,降低中医证候积分,临床疗效显著。Objectiye To study the therapeutic effect of Jianpi Zhixie Decoction combined with massage in the treatment of diarrhea in children.Methods A total of 88 children with diarrhea were selected and randomly divided into an observation group and a control group,with 44 cases in each group.The control group was given the western medicine symptomatic treatment,while the observation group was given Jianpi Zhixie Decoction combined with massage on the basis of that,both for 3 consecutive days.The curative effect of the two groups after 3 days of treatment was observed,the time of clinical symptom regression of the two groups was recorded,and the TCM syndrome scores,the levels of immunological indexes and serum inflammatory factors were compared between the two groups before and after treatment for 3 days.Results After 3 days of treatment,the total effective rate in the observation group(97.73%,43/44)was higher than that in the control group(81.82%,36/44)(P<0.05).After 3 days of treatment,the scores of stool characteristics,stool frequency,abdominal pain,dysphoria,vomiting,thirst,and the serum levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in both groups were lower than those before treatment,and the observation group was lower than the control group(P<0.05).The time of relieving abdominal pains,stopping diarrhea,resuming eating and correcting dehydration in the observation group was shorter than that in the control group(P<0.05).After 3 days of treatment,the levels of peripheral blood CD4^(+)/CD8^(+),serum immunoglobulin A(IgA),immunoglobulin G(IgG)and interleukin-10(IL-10)in the two groups were higher than those before treatment,and the observation group was higher than the control group(P<0.05).Conclusion Jianpi Zhixie Decoction combined with massage can improve the levels of peripheral blood D4^(+)/CD8^(+),serum IgA and IgG,adjust the levels of serum inflammatory factors,effectively improve the immune function of the body,and then promote the regression of clinical symptoms and reduce the TCM syndrom
