不同方法治疗晚期视网膜母细胞瘤对VEGF与MMP9表达的影响

摘　　要：目的探讨两种不同的方法治疗晚期视网膜母细胞瘤(RB)对血管内皮生长因子(VEGF)、基质金属蛋白酶9(MMP9)表达的影响。方法回顾性病例对照研究。收集郑州大学附属儿童医院2018年1月至2021年1月收治的儿童晚期(D期和E期)RB 62例(62眼)的临床资料。根据治疗方法分为对照组(32例)和试验组(30例)。对照组接受经眼动脉灌注化学药物治疗(IAC+局部治疗),试验组接受经静脉全身化学药物治疗(VEC方案)+IAC+局部治疗。比较两组患儿治疗效果、肿瘤直径、肿瘤厚度、血清VEGF、MMP9及不良反应。结果试验组治疗效果明显优于对照组(Z=-5.39,P<0.001)。两组治疗后肿瘤直径、肿瘤厚度均低于治疗前水平(均P<0.05),治疗后试验组肿瘤直径、肿瘤厚度均低于对照组(t=11.34、10.23,均P<0.001),试验组肿瘤直径、肿瘤厚度治疗前后差值均明显大于对照组(t=-3.63、-2.94,P=0.001、0.005)。治疗后两组血清VEGF及MMP9均低于治疗前水平(P<0.05),治疗后试验组血清VEGF及MMP9均低于对照组(t=6.21、8.44,均P<0.001),试验组血清VEGF及MMP9治疗前后差值均大于对照组患儿(t=-5.13、-5.60,均P<0.001)。两组患儿不良反应总发生率比较,差异无统计学意义(χ^(2)=0.57,P=0.450)。结论IVC+IAC+局部治疗晚期(D期及E期)RB患儿安全有效,并抑制VEGF及MMP9表达。Objective To study the efficacy of two methods in the treatment of children with advanced retinoblastoma(RB)on the expression of vascular endothelial growth factor(VEGF)and matrix metalloproteinase 9(MMP9).Methods This was a retrospective case-control study.The clinical data of 62 eyes of 62 children with late-stage(D and E)RB in the Children’s Hospital of Zhengzhou University from Jan.2018 to Jan.2021 were analysed.Based on the treatment method,they were divided into the control group(32 cases)and the trial group(30 cases).The control group received intraocular artery infusion chemotherapy(IAC)+local treatment,and the trial group received intravenous systemic chemotherapy(VEC regimen)+IAC+local treatment.The treatment efficacy,tumor diameter,tumor thickness,the expression of serum VEGF,MMP9 and adverse reactions were compared between the two groups.Results The treatment efficacy of the trial group was significantly better than that of the control group,and the difference was statistically significant(Z=-5.39,P<0.001).The tumor diameter and tumor thickness of the two groups after treatment were lower than those before treatment(all P<0.05).After treatment,the tumor diameter and tumor thickness of the trial group were lower than those of the control group(t=11.34,10.23;all P<0.001),the differences of tumor diameter and tumor thickness in the trial group before and after treatment were significantly greater than those in the control group(t=-3.63,-2.94;P=0.001,0.005).After treatment,the expression of serum VEGF and MMP9 in the two groups were lower than those before treatment(P<0.05).And the serum VEGF and MMP9 in the trial group were lower than those in the control group(t=6.21,8.44;all P<0.001).The differences before and after treatment of serum VEGF and MMP9 in the trial group were greater than those in the control group(t=-5.13,-5.60;all P<0.001).There was no significant difference in the total incidence of adverse reactions between the two groups(χ^(2)=0.57,P=0.450).Conclusion IVC+IAC+local treatment for la
