摘  要:目的探讨眼后房结构、水平沟到沟间距2与水平角膜直径差异(ΔhSTS2-WTW)对ICL植入术后拱高的影响。方法回顾性病例系列研究。分析山西省眼科医院2018年9月至2020年3月行V4c型ICL植入术103例(203眼)的临床资料。患者术前屈光状态为等效球镜度(-9.32±2.68)D。应用多元线性回归分析术后拱高的影响因素,并根据ΔhSTS2-WTW不同分组后进一步分析对拱高的影响及组间参数的差异。结果术后1个月拱高为124~1180(504.47±188.65)μm,其中非理想拱高30眼(14.78%)。结果提示ΔhSTS2-WTW(β=-0.27,95%CI:-211.50~-65.27,P<0.001)、晶状体矢高(CLR)(β=-0.22,95%CI:-363.18~-78.17,P=0.003)、前房容积(ACV)(β=0.17,95%CI:0.11~2.01,P=0.030)是影响术后拱高的独立因素。分组为:1组ΔhSTS-WTW<-0.1;2组-0.1≤ΔhSTS-WTW<0.3;3组ΔhSTS-WTW≥0.3,3组间非理想拱高比例(χ^(2)=7.77,P=0.020)、拱高(F=11.01,P<0.001)、平均后房角角度(PCA)(F=3.89,P=0.022)及睫状体厚度(CBT)(F=6.26,P=0.002)差异有统计学意义,而内前房深度(iACD)、前房角角度(ACA)、ACV及CLR差异均无统计学意义(均P>0.05)。结论有晶状体眼人工晶状体植入术前眼内沟到沟与眼表白到白差异较大时ICL植入术后可能出现非理想拱高,可以结合后房结构选择ICL尺寸,有助于获得理想拱高。Objective To investigate the effects of preoperative biometric parameters of the posterior chamber and the difference between the horizontal ciliary sulcus-to-sulcus 2(hSTS2)and horizontal corneal diameter(WTW)diameters(ΔhSTS2-WTW)on the vault after implantable collamer lens(ICL)implantation in phakic eye.Methods This was a retrospective case series study.The clinical data of 203 eyes of 103 patients who underwent V4c ICL implantation from Sep.2018 to Mar.2020 in Shanxi Eye Hospital were analyzed retrospectively.The equivalent spherical powers of preoperative refractive state were(-9.98±2.80)D.Multiple linear regression was used to analyze the influencing factors of postoperative vault,and further analyzed the influence on vault and the difference of parameters between groups according to the difference ofΔhSTS2-WTW.Results The vaults were 124-1180(504.47±188.65)μm at 1 month postoperatively.Among them,the non-ideal vaults accounted for 30 eyes(14.78%).Multiple linear regression analysis indicated thatΔhSTS2-WTW(β=-0.27,95%CI:-211.50--65.27,P<0.001),crystalline lens rise(CLR)(β=-0.22,95%CI:-363.18--78.17,P=0.003)and anterior chamber volume(ACV)(β=0.17,95%CI:0.11~2.01,P=0.030)were independent factors affecting vault.According to the difference ofΔhSTS2-WTW,these cases were divided into three groups:Group 1,ΔhSTS-WTW<-0.1;Group 2,-0.1≤ΔhSTS-WTW<0.3;Group 3,ΔhSTS-WTW≥0.3.There were statistically significant differences in the proportion of non-ideal vault(χ^(2)=7.77,P=0.020)and the postoperative vaults(F=11.01,P<0.001)among the three groups.The difference of mean posterior chamber angle(F=3.89,P=0.022)and ciliary body thickness(F=6.26,P=0.002)were statistically significant,while the difference of internal anterior chamber depth,anterior chamber angle,ACV and CLR were not statistically significant(all P>0.05).Conclusion The difference between the WTW and STS diameters may be associated with an undesirable vault after ICL implantation in phakic eye.The ICL size can be determined in combination with t
