摘  要:目的探讨子宫内膜癌微卫星稳定性检测中不同方法结果差异的原因。方法收集广州医科大学附属第二医院和南方医科大学南方医院子宫内膜癌标本242例,采用免疫组化(IHC)染色检测错配修复(MMR)蛋白,PCR-毛细管电泳法(PCR-CE)进行微卫星不稳定性(MSI)基因片段分析,探讨两种方法检测结果不匹配的原因。结果IHC染色和PCR-CE检测结果的不一致率为7.02%(P<0.01)。10/36的MLH1/PMS2蛋白缺失子宫内膜癌患者表现为微卫星稳定(MSS),3/8的MSH6蛋白缺失子宫内膜癌患者表现为MSS/MSI-L。结论伴有乳腺癌相关病史、家族史的MSH6蛋白单缺失的早期子宫内膜癌患者,在微卫星稳定性检测中,IHC染色比PCR-CE检测敏感性更高,IHC染色结果更可靠。Objective To investigate the reason for the difference of microsatellite stability test results in endometrial carcinomas(ECs).Methods 242 cases of ECs were collected.The mismatch repair(MMR)protein was detected by immunohistochemistry(IHC)and microsatellite instability(MSI)was detected by PCR reactions and capillary electrophoresis(PCR-CE).The causes of inconsistency between IHC staining and PCR-CE detection were analyzed.Results The discordance rate of IHC staining and PCR-CE detection was 7.02%(P<0.01).Microsatellite stability(MSS)was found in 10 of 36 MLH1/PMS2 deficient ECs and MSS/MSI-L was found in 3 of 8 MSH6 deficient ECs.Conclusion In early endometrial cancer patients with breast cancer-related medical history or family history of MSH6 deficiency,IHC staining is more sensitive and reliable than PCR-CE detection in microsatellite stability testing.
