胃血管球瘤4例临床病理分析及文献复习

摘　　要：目的探讨胃血管球瘤(gastric glomus tumor,GGT)的临床病理学特征、鉴别诊断、治疗与预后。方法回顾性分析4例胃血管球瘤患者的临床病理资料,总结其临床病理学特点、免疫组化等,并复习相关文献。结果4例胃血管球瘤患者均为女性,中位年龄55岁。4例患者均有明显的临床症状,主要表现为恶心呕吐、间断性腹部不适,胀痛、纳差,1例病人出现间断性黑便。组织学上,肿瘤细胞呈实性片状生长,分布于血管之间或围绕血管周围,血管扩张。瘤细胞呈圆形,胞浆透明或嗜酸性,细胞核圆形,染色质细腻,核仁模糊,核分裂<1个/50HPF。免疫组化显示,肿瘤细胞表达Vimentin、SMA、calponin、H-caldesmon、Ⅳ型胶原。结论胃血管球瘤是一种胃肠道少见的具有血管周围血管球体细胞特点的间叶性肿瘤,好发于中老年女性,预后较好,结合临床和组织病理学特征及免疫组化标记,可做出准确的病理诊断。Objective To investigate the clinicopathological features,differential diagnosis,treatment and prognosis of gastric glomus tumor.Methods The clinicopathological data of 4 patients with gastric glomus tumor were retrospectively analyzed,and their histopathological features,immunohistochemistry and so on were concluded,and the related documents were reviewed.Results In the 4 gastric glomus tumor patients,all of the patients were females,and the median age of onset was 50 years.Four patients had obvious clinical symptoms,mainly nausea and vomiting,intermittent abdominal discomfort,distending pain and anorexia.One patient had intermittent black stool.Typical gastric glomus tumor had a characteristic pathological pattern:the tumor cells were solid growth,arranged in an organ-like manner around the hemangiopericytoma-like vessels,and common vasodilation.The cells were round with transparent or eosinophilic cytoplasm and fine chromatin;the nucleolus was fuzzy,and the mitosis<1/50 HPF.Immunohistochemical results showed that vimentin,SMA,calponin,H-caldesmon and collagen 4 were all positive.Conclusion Gastric glomus tumor is a rare mesenchymal tumor with the characteristics of perivascular glomus cells in the gastrointestinal tract.It occurs in middle aged and elderly women and the final prognosis is better.Combined with clinical and histopathological characteristics,accurate pathological diagnosis can be made.
