摘　　要：目的探讨肺纤毛黏液结节性乳头状肿瘤(CMPT)/细支气管腺瘤(BA)的临床病理特征。方法对4例肺CMPT/BA病例进行回顾性临床病理分析,加做免疫组化及基因检测,并结合文献复习。结果4例中男性2例,女性2例,年龄35~65岁。临床症状无特殊,肿瘤指标无异常,均为体检发现肺部肿块,光镜下见纤毛柱状细胞、黏液细胞和基底细胞,排列成腺样或乳头状结构。免疫组化显示上皮细胞TTF1、CK7等阳性,基底细胞P63、CK5/6等阳性。基因检测2例存在BRAFV600E突变,2例无突变。随访2~67个月,均无复发或转移等。结论肺CMPT/BA少见,患者无明显临床症状,多为体检发现,确诊依靠病理形态及免疫组化,手术切除后无特殊辅助治疗,文献报道均未见复发转移等。Objective To analyze the clinical and pathological features of ciliated muconodular papillary tumor(CMPT)/bronchiolar adenoma(BA)of the lung.Methods We retrospectively summarized the clinicopathological data of the 4 CMPT/BA cases,and their immunophenotype and gene detection were analyzed.We performed a comprehensive literature search for published articles.Results Of the 4 patients,2 were man and 2 were women,aged 35 to 65 years.Four cases were discovered during health care screenings.The pathological results showed that the nodule was composed of proliferated ciliated cells,mucous cells and basal cells.The epithelial cells were covered on the surface of glandular and papillary structures.The immunohistochemical staining showed that TTF1 and CK7 were positive in epithelial cells;P63 and CK5/6 were positive in basal cells.We identified BRAF V600E mutation in two cases.The cases were followed up for 3-55 months,showing that there was no recurrence and metastasis.Conclusion The pulmonary CMPT/BA is a very rare tumor.According to its typical histopathological characteristics and immunohistochemical phenotype,it can be differentiated from other pulmonary diseases.Surgical resection for the tumor reveals a good prognosis.
