经乳晕麦默通微创旋切术治疗乳腺纤维瘤患者的临床效果

摘　　要：目的:分析经乳晕麦默通微创旋切术治疗乳腺纤维瘤患者的临床效果。方法:回顾性选取昆山市第二人民医院2019年8月-2021年8月收治的74例乳腺纤维瘤患者的临床资料。根据选择的不同术式将其分为参照组(34例)和研究组(40例)。参照组给予放射状切口切除手术治疗,研究组给予经乳晕麦默通微创旋切术治疗。比较两组围手术期指标、并发症、术后3个月乳晕区感觉神经情况,术前1 d、术后3个月泌乳素(PRL)、雌二醇(E_(2))。结果:研究组手术时间、切口愈合时间、切口长度、瘢痕长度均短于参照组,术中出血量少于参照组(P<0.05)。研究组术后并发症发生率低于参照组(P<0.05)。研究组乳晕区感觉神经正常率高于参照组(P<0.05)。术后,两组E_(2)、PRL水平均低于术前,研究组E_(2)、PRL水平均低于参照组(P<0.05)。结论:乳腺纤维瘤患者采用经乳晕麦默通微创旋切术治疗的出血量少,瘢痕小,效果显著,可以减少并发症的发生,减轻对乳晕区感觉神经的损伤,改善雌激素水平。Objective:To analyze the clinical effect of Mammotome minimally invasive circumcision via areola in the treatment of patients with breast fibroma.Method:The clinical data of 74 patients with breast fibroma treated in Kunshan Second People’s Hospital from August 2019 to August 2021 were retrospectively selected.The patients were divided into the reference group(34 cases)and the study group(40 cases)according to the different surgical methods.The reference group was given the radial incision resection,and the study group was given the Mammotome minimally invasive circumcision via areola.The perioperative indexes,complications,sensory nerves condition in the areola area 3 months after operation,prolactin(PRL)and estradiol(E_(2))1 day before operation and 3 months after operation were compared between the two groups.Result:The operation time,wound healing time,wound length and scar length of the study group were shorter than those of the reference group,and the amount of intraoperative bleeding was less than that of the reference group(P<0.05).The incidence of postoperative complications in the study group was lower than that in the reference group(P<0.05).The normal rate of sensory nerve in the areola area in the study group was higher than that in the reference group(P<0.05).After operation,E_(2) and PRL levels in the two groups were lower than those before operation,and E_(2) and PRL levels in the study group were lower than those in the reference group(P<0.05).Conclusion:The patients with breast fibroma treated with Mammotome minimally invasive circumcision via areola have less bleeding,less scar and significant effect.It can reduce the occurrence of complications,reduce the damage to the sensory nerve in the areola area and improve the estrogen level.
