摘 要:目的:探究不同剂量多层螺旋CT检查在肺内病变筛查中的诊断效能及图像质量。方法:回顾性分析2019年12月-2020年12月厦门大学附属第一医院杏林分院收治疑似肺内病变的130例患者的临床资料,均行多层螺旋CT检查,按照剂量分为观察组(n=65,低剂量多层螺旋CT)和对照组(n=65,普通剂量多层螺旋CT),对比两组检查结果、结节检出情况(结节个数、结节最小直径、结节最大直径、结节平均直径)、辐射剂量[CT剂量指数(CTDIvol)、总毫安秒、剂量长度乘积(DLP)、放射剂量]、图像质量[图像质量评分、CT图像的信号噪声比(SNR)]。结果:常规剂量多层螺旋CT诊断肺内病变的敏感度为88.89%,特异度为85.00%,准确率为87.69%,阳性预测值为93.02%,阴性预测值为77.27%,Kappa值为0.719。低剂量多层螺旋CT诊断肺内病变的敏感度为87.50%,特异度为88.24%,准确率为87.69%,阳性预测值为95.45%,阴性预测值为71.43%,Kappa值为0.704。两组结节个数、结节最大直径、结节平均直径比较差异无统计学意义(P>0.05),观察组结节最小直径明显小于对照组(P<0.05);观察组CTDIvol、总毫安秒、DLP、放射剂量均明显低于对照组(P<0.05);两组图像质量评分和SNR比较差异无统计学意义(P>0.05)。结论:常规剂量和低剂量多层螺旋CT在肺内病变筛查中均具有良好诊断效能,并且不影响图像质量,但低剂量多层螺旋CT可降低辐射剂量,更容易发现直径较小的结节。Objective:To explore the diagnostic efficacy and image quality of multislice spiral CT with different doses in screening pulmonary lesions.Method:The clinical data of 130 patients with suspected pulmonary lesions admitted to Xinglin Branch of the First Affiliated Hospital of Xiamen University from December 2019 to December 2020 were retrospectively analyzed.All patients underwent multi-slice spiral CT examination,and were divided into observation group(n=65,low-dose multi-slice spiral CT)and control group(n=65,normal-dose multi-slice spiral CT)according to dose.Compared the test result,nodule detection(nodule number,nodule minimum diameter,nodule maximum diameter,nodule average diameter),radiation dose[CT dose index(CTDIvol),the total milliampere seconds,dose length product(DLP),radiation dose],image quality[image quality score and CT image signal to noise ratio(SNR)]of two groups.Result:The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of conventional dose multi-slice spiral CT in the diagnosis of pulmonary lesions were 88.89%,85.00%,87.69%,93.02% and 77.27% respectively,and the Kappa value was 0.719.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of low-dose multi-slice spiral CT in the diagnosis of pulmonary lesions were 87.50%,88.24%,87.69%,95.45% and 71.43% respectively,and the Kappa value was 0.704.There were no significant differences in the number,maximum diameter and average diameter of nodules between the two groups(P>0.05),and the minimum diameter of nodules in the observation group was significantly smaller than that in the control group(P<0.05).CTDIvol,total milliampere second,DLP and radiation dose in observation group were significantly lower than those in control group(P<0.05).There were no significant differences in image quality score and SNR between the two groups(P>0.05).Conclusion:Both conventional dose and low-dose multi-slice spiral CT have good diagnostic efficacy in the screening of pulmonary lesions,a
