苯溴马隆致药物性肝衰竭1例

摘　　要：高尿酸血症的发病逐年升高并呈现年轻化。长期的高尿酸可加速血管损伤、硬化,从而增加心脑血管意外、肾功能不全等疾病的发生风险,得到了临床上越来越多的关注。治疗上除了改善生活方式,降尿酸药物也得到了很大的发展。现临床中使用广泛的降尿酸药物为苯溴马隆,其效果得到了临床的肯定,且使用简便、价格实惠,患者依从性好。其主要不良反应有胃肠道反应、变态反应和肝毒性。本研究中患者为一高龄男性,因高尿酸血症使用苯溴马隆治疗后出现肝衰竭,于停药、保肝后肝功能恢复,警示临床中老年人降尿酸治疗时更需要注重药物选择和随访。The incidence of hyperuricemia is increasing year by year,and the age of patients is becoming younger.Long-term hyperuricemia can accelerate vascular injury and sclerosis,thus increasing the risk of cardiovascular and cerebrovascular accidents,renal insufficiency and other diseases,which has attracted more and more attention in clinic.In addition to improving the lifestyle in the treatment,uric acid-lowering drugs have also been greatly developed.At present,benzbromarone,a widely used uric acid-lowering drug in clinic,has been clinically affirmed in its efficacy.Meanwhile,it is easy and convenient to use,its price is affordable,and the patient's compliance is good.The main adverse reactions are gastrointestinal reactions,allergic reactions and hepatotoxicity.The patient in this paper is an elderly male,who developed hepatic failure after treatment with benzbromarone for hyperuricemia,and his hepatic function recovered after drug withdrawal and hepatic protection.It warns that middle-aged and elderly people s hould pay more attention to drug s election and follow-up in clinical uric acid lowering treatment.
