摘　　要：目的:验证帕金森病认知功能评分量表(PD-CRS)的信效度,探索区分帕金森病(PD)病人认知正常(PD-NC)、轻度认知障碍(PD-MCI)和痴呆(PDD)的筛查界值,评价PD-CRS在帕金森病认知障碍(PD-CI)病人筛查中的应用效果。方法:在170例PD病人中进行问卷调查,检验PD-CRS的信效度,绘制ROC曲线确定最佳界值。结果:PD-CRS共有9个条目,探索性因子分析提取1个公因子,累计方差贡献率68.718%。量表Cronbach′sα系数为0.868,重测信度为0.985,差异均具有统计学意义(P<0.001)。以<76分作为PD-MCI的最佳界值;以<68分作为PDD的最佳临界值。结论:PD-CRS用于评估PD认知功能具有较高的信效度,是有效的特异性筛查工具,对PD-CI各阶段的诊断价值较高。Objective:To verify the reliability and validity of Parkinson′s Disease Cognitive Rating Scale(PD-CRS),explore the screening cut-off value for distinguishing cognitive normality(PD-NC),mild cognitive impairment(PD-MCI)and dementia(PDD)in patients with Parkinson′s disease(PD)and to evaluate the application effect of PD-CRS in the screening of Parkinson′s disease with cognitive impairment(PD-CI).Methods:The reliability and validity of PD-CRS were tested by questionnaire in 170 patients with PD,and the ROC curve was drawn to determine the best critical value.Results:There were 9 items in PD-CRS,and one common factor was extracted by exploratory factor analysis,and the cumulative variance contribution rate was 68.718%.Cronbach′sαcoefficient was 0.868 and test-retest reliability was 0.985,and the differences were statistically significant(P<0.001).The best cut-off value of PD-MCI was<76.A score of<68 was used as the optimal cut-off value for PDD.Conclusion:PD-CRS has high reliability and validity in the assessment of PD cognitive function,and is an effective and specific screening tool.It has better diagnostic value for all stages of PD-CI.
