摘 要:目的研究分析成人安氏Ⅱ类1分类下颌后缩高角型患者颅颌面和上气道的形态结构。方法选取2019年6月至2021年8月就诊于郑州大学第一附属医院正畸科的成人安氏Ⅱ类1分类下颌后缩高角型患者和安氏Ⅰ类高角型患者,各30例。对每例患者拍摄头颅侧位定位片,对上下颌骨、上气道进行测量,分析两组间的差异。结果在颌骨测量中,相对于安氏Ⅰ类高角组,安氏Ⅱ类1分类下颌后缩高角组上下颌骨相对位置(ANB角)、颌凸角(NA-PA)、下颌位置(S-Co)增大(P<0.05),下颌骨相对于颅骨的位置(SNB角)、下颌体长(Go-Po)、下颌升支长(Co-Go)、下颌综合长度(Co-Po)、后面高/前面高(S-Go/N-Me)减小,差异有统计学意义(P<0.05);上气道测量中,相对于安氏Ⅰ类高角组,安氏Ⅱ类1分类下颌后缩高角组腭咽气道间隙(U-MPW)、舌咽气道间隙(TB-TPPW)减小,差异有统计学意义(P<0.05)。结论成人安氏Ⅱ类1分类下颌后缩高角型患者下颌体及下颌升支短小,下颌后缩、后旋。成人安氏Ⅱ类1分类下颌后缩高角型患者上气道存在明显狭窄,且狭窄段多位于腭咽段和舌咽段。Objective To analyze the morphology of craniomaxillofacial and upper airway in adult Angle’s class Ⅱ division 1 with mandibular retraction and high angle patients.Methods The adult Angle’s class Ⅱ division 1 with mandibular retraction and high angle patients and adult Angle’s class Ⅰ with high angle patients were selected from June 2019 to August 2021 in the First Affiliated Hospital of Zhengzhou University,each 30 cases.Digital X-rays cephalometric photographs were taken for each patient,and the upper and lower jaws,upper airways were measured to analyze the differences between two groups.Results In jaw measurement,compared with the Angle Ⅰ group,relative position of jaw bone(ANB angle),angle of conrexity(NA-PA),the lower jaw position(S-Co)in Angle’s class Ⅱ division 1 with mandibular retraction and high angle group were increased(P<0.05).The position of the mandible relative to the skull(SNB angle),length of the mandibular body(Go-Po),mandibular ascending ramus(Co-Go),mandibular length(Co-Po),posterior face height/anterior face height(S-Go/N-Me)were decreased,and the differences were statistically significant(P<0.05).In upper airway measurement,compared with the Angle Ⅰ group,pharynx space(U-MPW)and palatopharynx space(TB-TPPW)in Angle’s class Ⅱ division 1 with mandibular retraction and high angle group were decreased,and the differences were statistically significant(P<0.05).Conclusion The mandibular body and mandibular ramus are short in adult Angle’s class Ⅱ division 1 with mandibular retraction and high angle patients.There is significant narrowing of the upper airway in adult Angle’s class Ⅱ division 1 with mandibular retraction and high angle patients,and the more narrow part is in tongue pharynx and palatopharynx.
