摘　　要：目的比较分析不同免疫球蛋白分型MM患者的实验室数据差异,为MM分型提供辅助诊断。方法收集2017年1月至2021年12月在我院确诊的100例初发MM患者(其中96例为3种常见分型)的临床特征、血常规、血清生化检测结果,通过分析96例常见型MM患者的临床特征,并比较血常规与血清生化指标,分析出具有统计学意义的指标。结果96例常见分型MM为IgG型、IgA型及轻链型,以骨痛和乏力最多见,3种常见分型的MM至少有两种分型在RBC(P<0.05)、LYM#(P<0.05)、LDL-C(P<0.05)、ALP(P<0.05)和CA(P<0.05)上的差异存在统计学意义,在ALB(P<0.001)、GLB(P<0.001)、LDH(P<0.01)和TC(P<0.01)上的差异也存在统计学意义。两两多重比较,IgG型与轻链型之间的TC水平差异具有统计学意义(P<0.01);轻链型与IgA型(P<0.05)、轻链型与IgG型之间的LDL-C水平差异具有统计学意义(P<0.01);IgG型与轻链型之间的CA水平差异具有统计学意义(校正后P<0.05);IgA型与轻链型之间的LDH差异具有统计学意义(校正后P<0.01);IgA型,IgG型和轻链型3种分型之间的ALB和GLB指标差异具有统计学意义(P<0.01)。结论早期不同分型的MM在血清蛋白及血脂指标上差异具有统计学意义,对早期MM的分型有一定的参考价值。Objective The laboratory data of patients with different immunoglobulin types of MM were compared and analyzed to provide an auxiliary diagnosis for MM typing.Methods The clinical characteristics,blood routine and serum biochemical test results of 100 patients with initial MM diagnosed in our hospital from January,2017 to December,2021 were collected for the study.By analyzing the clinical characteristics of 96 patients with common MM,and comparing the blood routine and serum biochemical indicators among patients,the indicators with statistical significance were identified.Results The common MM types of 96 cases were IgG,IgA and light chain types,while bone pain and fatigue were the most common conditions.There were statistically significant differences in RBC(P<0.05),LYM#(P<0.05),LDL-C(P<0.05),ALP(P<0.05)and CA(P<0.05)among the three common MM types.There were also significant differences in ALB(P<0.001),GLB(P<0.001),LDH(P<0.01)and TC(P<0.01).There was a significant difference in TC content between IgG type and light chain type(P<0.01).There were statistically significant differences in LDL-C content between light chain type and IgA type(P<0.05),light chain type and IgG type(P<0.01),respectively.There was a significant difference in CA concentration between IgG type and light chain type(P<0.05 after correction).There was a significant difference in LDH between IgA type and light chain type(P<0.01 after correction).There were significant differences in ALB and GLB(P<0.01)among IgA,IgG and light chain types.Conclusion There is a statistical significance in serum lipid and protein of MM with different early classifications,which has a certain reference value for the early classification of MM.
