摘 要:目的:探讨血浆锰超氧化物歧化酶(MnSOD)Ala-9Val多态性与精神分裂症发病及认知功能异常的相关性。方法:选取434例符合精神障碍诊断与统计手册第4版(DSM-Ⅳ)精神分裂症诊断标准的患者和322例正常对照,采用重复性成套神经心理状态测验(RBANS)测查认知功能,采用聚合酶链反应及限制性片段长度多态性方法检测MnSOD Ala-9Val多态性。结果:精神分裂症患者和正常对照MnSOD Ala-9Val基因型及等位基因频率均无统计学意义(均P>0.05)。RBANS测试中,除视觉广度(P>0.05)外,患者组的即刻记忆、言语功能、注意力、延时记忆及总分均低于正常对照组(均P<0.01)。基因型和诊断组别的交互作用对言语功能因子分有统计学意义(P<0.01),进一步分析发现患者组中携带Ala等位基因的患者言语功能因子分高于Val纯合子患者(P<0.01)。结论:MnSOD Ala-9Val多态性可能不是精神分裂症的易感因素,但Ala等位基因与精神分裂症患者的言语功能受损可能存在相关。Objective:To investigate the effects of MnSOD-gene Ala-9 Val polymorphism on the onset of schizophrenia and its cognitive impairment.Methods:Totally 434 patients with schizophrenia who met the diagnostic criteria of the Diagnostic and Statistical Manual of Mental Disorders,Fourth Edition(DSM-IV)and 322 normal controls were recruited.The Repeatable Battery for the Assessment of Neuropsychological Status(RB ANS)was adopted for cognitive function.Polymerse chain reaction and restriction fragment length polymorphism were used for MnSOD Val-9 Ala polymorphism.Results:There were no significant differences in MnSOD Val-9 Ala genotype and allele frequency between the patient and the normal control groups(P>0.05).In the RBANS test,the scores of immediate memory,language,attention,delayed memory and the total scores were lower in the schizophrenia group than in the normal control group(P<0.01)except for the sores of visuospatial/constructional index(P>0.05).The interaction between genotype and diagnosis had a statistically significant effect on language index scores(P<0.01).Further analysis found that patients carrying the Ala allele had significantly higher language scores than those carrying the Val homozygote(P<0.01).Conclusion:It suggests that the MnSOD Ala-9 Val polymorphism may not be a susceptibility factor for schizophrenia,but the Ala allele may be significantly associated with impaired language function in patients with schizophrenia.
