^(18)F-FDG PET/CT在子宫内膜癌术前诊断中的作用

摘　　要：目的评价^(18)F-FDG PET/CT在子宫内膜癌术前诊断中的作用.方法回顾性分析2016-03-01-2019-09-30在梅州市人民医院术前行^(18)F-FDG PET/CT检查的120例疑似子宫内膜癌患者的显像结果,以患者术后的病理结果、前期的临床病例资料以及后续随访情况作为基础,针对性地探讨子宫内膜癌患者的原发灶和转移灶^(18)F-FDG PET/CT显像的价值与作用.结果120例疑似子宫内膜癌患者,^(18)F-FDG PET/CT显像阳性118例,阴性2例,假阳性3例,假阴性2例.^(18)F-FDG PET/CT显像对子宫内膜癌原发灶诊断的灵敏度为98.3%(115/117),准确度为95.8%(115/120);对子宫内膜癌淋巴结转移灶诊断的灵敏度为95.0%(19/20),特异度为89.7%(87/97),准确度为90.6%(106/117);对子宫内膜癌远处转移灶诊断的灵敏度为92.3%(12/13),特异度为99.0%(103/104),准确度为98.3%(115/117).另外,^(18)F-FDG PET/CT显像发现10例患者合并双原发癌或多原发癌,均经病理确诊.结论^(18)F-FDG PET/CT显像对子宫内膜癌患者术前诊断具有较高的灵敏度和特异度,对及时检出双原发癌或多原发癌具有重要价值.Objective To evaluate the role of^(18)F-FDG PET/CT in the preoperative diagnosis of endometrial cancer.Methods The imaging results of 120 patients with suspected endometrial cancer who underwent preoperative^(18)F-FDG PET/CT examination in Meizhou People’s Hospital on 2016-03-01-2019-09-30 were retrospectively analyzed, and the postoperative pathological results were used to analyze the imaging results of the patients.Based on the clinical case data in the early stage and the follow-up situation, the value and role of^(18)F-FDG PET/CT imaging in the primary and metastatic lesions of endometrial cancer patients were discussed in a targeted manner.Results Among 120 patients with suspected endometrial cancer, 118 cases were positive, 2 cases were negative, 3 cases were false positive, and 2 cases were false negative by^(18)F-FDG PET/CT.The sensitivity of^(18)F-FDG PET/CT imaging for the diagnosis of primary endometrial cancer was 98.3%(115/117),and the accuracy was 95.8%(115/120).The sensitivity for the diagnosis of endometrial cancer lymph node metastases was 95.0%(19/20),the specificity was 89.7%(87/97),and the accuracy was 90.6%(106/117).The sensitivity for the diagnosis of distant metastases in endometrial cancer was 92.3%(12/13),with a specificity of 99.0%(103/104) and an accuracy of 98.3%(115/117).In addition,^(18)F-FDG PET/CT imaging revealed 10 patients with dual or multiple primary cancers, all of which were pathologically confirmed.Conclusion^(18)F-FDG PET/CT imaging has high sensitivity and specificity for the preoperative diagnosis of endometrial cancer patients, and is of great value for the timely detection of dual primary cancers or multiple primary cancers.
