摘  要:目的探讨火针对坐骨神经损伤大鼠神经功能及Nrg1/ErbB2信号通路的影响。方法将54只雄性SpragueDawley大鼠随机分为假手术组、模型组、火针组,每组18只。模型组、火针组大鼠建立坐骨神经钳夹损伤模型。建模成功24 h后,火针组采用火针针刺大鼠右侧后肢环跳穴、委中穴,隔天1次,共干预14 d;假手术组与模型组仅进行与火针组相同的捆绑束缚,不进行其他处理。分别于干预1 d、7 d、14 d后对各组大鼠进行行为学检测,记录坐骨神经功能指数(SFI);然后各时间点各组随机处死6只大鼠,取损伤处坐骨神经,电镜观察损伤处超微结构的变化,免疫组化观察坐骨神经组织中Nrg1、ErbB2阳性表达情况。结果火针组与模型组干预1 d、7 d、14 d后SFI均明显低于假手术组(P均<0.05);火针组干预7 d、14 d后SFI均明显高于同期模型组(P均<0.05),且火针组干预14 d后SFI明显高于干预7 d后(P<0.05)。干预14 d后,电镜下模型组有少量髓鞘再生,线粒体等细胞器结构趋于正常;火针组出现大量新生髓鞘,线粒体等细胞器结构正常。干预1 d后,各组大鼠坐骨神经组织中Nrg1阳性表达光密度值比较差异均无统计学意义(P均>0.05),模型组、火针组ErbB2阳性表达光密度值均明显低于假手术组(P均<0.05);干预7 d、14 d后,火针组Nrg1、ErbB2阳性表达光密度值均明显高于同期模型组(P均<0.05);干预14 d后,火针组Nrg1、ErbB2阳性表达光密度值与假手术组比较差异均无统计学意义(P均>0.05)。结论火针通过Nrg1/ErbB2信号通路促进髓鞘修复再生可能是火针促进坐骨神经损伤修复的分子机制之一。Objective It is to explore the effect of fire acupuncture on nerve function and Nrg1/ErbB2 signaling pathway in rats with sciatic nerve injury.Methods Fifty-four male SpragueDawley rats were randomly divided into sham operation group,model group and fire acupuncture group,with 18 rats in each group.Rat model of sciatic nerve clamp injury were established in the model group and fire acupuncture group.24 hours after successful modeling,the rats in the the fire acupuncture group were treated with fire acupuncture at Huantiao and Weizhong acupoints on the right hind limb,once every other day,treated for a total of 14 days;the rats in the sham operation group and the model group were treated with the same binding and restraint as the fire acupuncture group,and no other treatment was performed.After 1 d,7 d,and 14 d of intervention,behavioral testing was performed in the rats in each group,and their sciatic nerve function index(SFI)was recorded;then 6 rats in each group were randomly sacrificed at each time point,and the injured sciatic nerve was collected to observe the changes of microstructure by electric microscopy and the positive expression of Nrg1 and ErbB2 in sciatic nerve tissue by immunohistochemistry.Results The SFI of the fire acupuncture group and the model group after 1 d,7 d,and 14 d of intervention were significantly lower than those of the sham operation group(all P<0.05);the SFI of the fire acupuncture group after 7 d and 14 d of intervention were significantly higher than those of the model group at the same time period(all P<0.05),and the SFI of the fire acupuncture group after 14 days of intervention was significantly higher than that after 7 days of intervention(P<0.05).After 14 days of intervention,electron microscope observation showed that there was a small amount of remyelination,and the structure of mitochondria and other organelles tended to be normal in the model group,a large number of new myelin sheaths appeared,and the organelle structures such as mitochondria were normal in the fire acu
