摘 要:目的观察睑缘炎患者蠕形螨感染和角膜上皮树突状细胞的相关性。方法收集2020年3月—12月就诊于沧州市中心医院眼科门诊的50例(50只眼)睑缘炎患者为试验组,对照组为性别和年龄相匹配的健康志愿者50例(50只眼)。所有入组病人均进行裂隙灯显微镜检查,运用活体共聚焦显微镜(IVCM)对睑缘炎患者睫毛根部毛囊内的蠕形螨进行定量检测,观察记录角膜上皮树突状细胞密度。结果试验组和对照组蠕形螨感染阳性率为分别为92%和68%,2组蠕形螨感染阳性率差异有统计学意义(χ^(2)=9.000,P=0.003)。试验组平均每个眼睑感染蠕形螨数量(7.28±3.86)只,显著高于对照组(4.02±3.79)只,差异有统计学意义(t=4.260,P=0.000)。试验组角膜上皮树突状细胞密度(53.44±11.55)个/mm2,显著高于对照组(20.28±6.96)个/mm2,差异有统计学意义(t=17.388,P=0.000)。结论IVCM可以有效地对睑缘炎患者睫毛毛囊内的蠕形螨和角膜上皮树突状细胞进行无创、定量检测,为睑缘炎的诊断和治疗提供依据。OBJECTIVE To observe the correlation between demodex infection and corneal epithelial dendritic cells in patients with blepharitis.METHODS A total of fifty patients(fifty eyes)with blepharitis who visited the ophthalmic clinic of Cangzhou Central Hospital from March to December 2020 were collected as the experimental group,while fifty healthy volunteers(fifty eyes)matched in gender and age were collected as the control group.All enrolled patients were examined by slit lamp microscope,and in vivo confocal microscope(IVCM)was used to quantitatively detect the number of demodex in the lash follicles as well as observe and record the density of corneal epithelial dendritic cells.RESULTS The positive rate of demodex infection was 92%in the experimental group and 68%in the control group.There was a significant difference between the two groups(χ^(2)=9.000,P=0.003).The average number of demodex infected on each eyelid in the experimental group(7.28±3.86)was significantly higher than that in the control group(4.02±3.79),and the difference was statistically significant(t=4.260,P=0.000).The density of corneal epithelial dendritic cells in the experimental group(53.44±11.55/mm2)was significantly higher than that in the control group(20.28±6.96/mm2),and the difference was statistically significant(t=17.388,P=0.000).CONCLUSIONS IVCM can effectively detect the demodex and corneal epithelial dendritic cells in the eyelash follicles of patients with blepharitis in a non-invasive and quantitative manner,provide a basis for the diagnosis and treatment of blepharitis.
