胃的超声充盈检查与核磁水造影在进展期胃癌术前TNM分期的对比分析

摘　　要：目的:比较胃的超声充盈检查(GFUS)与核磁水造影(MRH)在进展期胃癌术前TNM分期的效果。方法:回顾性分析河北中石油中心医院2018年12月至2020年12月收治的经胃镜诊断拟行手术治疗并最终经病理确诊的90例初次确诊胃癌患者临床资料,所有患者均在术前1周行胃的GFUS和MRH检查,并行TNM分期评估,与术后病理分期比较,同时对两种检查方式联合评估与术后病理分期的效果进行评估。结果:GFUS对胃癌T1和T4期的检出率均优于MRH(P<0.05);GFUS对胃癌N3分期的检出率均优于MRH(P<0.05)。GFUS和MRH对胃癌M分期的检出率比较差异无统计学意义(P>0.05)。GFUS联合MRH诊断胃癌M0期、M1期的共同阳性率分别为100.00%、71.43%,GFUS联合MRH诊断胃癌M0期、M1期的非共同阳性率分别为0.00%、14.29%。结论:GFUS可较好诊断进展期胃癌患者TNM分期,且T1期、T4期、N3期及总体检出率优于MRH诊断,可为手术治疗提供较佳依据。Objective:To compare the efficacy of gastric filling ultrasonography(GFUS)and magnetic resonance hydrography(MRH)in preoperative TNM staging of advanced gastric cancer.Methods:A retrospective analysis was performed on the clinical data of 90 initially diagnosed gastric cancer patients hospitalized in Hebei Central Hospital of China Petroleum from December 2018 to December 2020.All patients were examined with gastric contrast ultrasound and MRH one week before surgery and were evaluated with TNM stage.The staging effect of single and combined application of gastric contrast ultrasound and MRH was evaluated according to the postoperative pathological staging.Results:Gastric contrast ultrasound performed superior to MRH in detecting the T1 and T4 stages of gastric cancer(P<0.05).The detection rate of gastric contrast ultrasound in the N3 stage of gastric cancer was higher than that of MRH(P<0.05).There was no significant difference between gastric contrast ultrasound and MRH in detecting the M stage of gastric cancer(P>0.05).The positive diagnostic rates of gastric contrast ultrasound combined with MRH were 100.00% and 71.43%,respectively;while the corresponding negative diagnostic rates were 0.00% and 14.29%.Conclusion:Gastric contrast ultrasound can better diagnose the TNM stage of patients with advanced gastric cancer,and the detection rates of it in T1,T4,N3,and overall stages are better than those of MRH diagnosis,which can provide a better basis for surgical treatment.
