精神分裂症患者血清炎症因子水平及其与代谢指标相关性

摘　　要：目的:探讨精神分裂症患者血清炎症因子水平及其与代谢指标的关系。方法:收集115例精神分裂症患者(病例组)及54名健康志愿者(对照组)的一般人口学资料,测定并比较两组代谢指标和血清中9种炎症因子水平[白介素(IL)-1β、IL-4、IL-6、IL-8、IL-10、IL-12、肿瘤坏死因子(TNF)-α、干扰素(IFN)-γ及趋化因子(CCL)-2];采用Speaman相关性分析方法分析病例组血清中炎症因子水平与代谢指标[BMI、腹围、臀围、血压、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、三酰甘油(TG)、总胆固醇(CHO)、低密度脂蛋白(LDL)及高密度脂蛋白(HDL)]的相关性。结果:病例组血清IL-1β、IL-4、IL-6,、IL-8、TNF-α及CCL-2水平明显高于对照组(P均<0.01);病例组血清IL-4水平与BMI(r=0.20,P=0.03)及TG(r=0.22,P=0.02)呈正相关;IL-8与FPG(r=-0.25,P=0.01)及HDL(r=-0.26,P=0.01)呈负相关;IL-6和TNF-α水平与HDL呈负相关(r=-0.21,P=0.03;r=-0.29,P<0.01);CCL-2与BMI(r=0.19,P=0.04)、腹围(r=0.21,P=0.03)、臀围(r=0.19,P=0.04)及TG(r=0.23,P=0.02)呈正相关。结论:精神分裂症患者存在多种炎症因子水平升高,与其代谢指标异常具有一定的相关性。Objective:To investigate the changes of serum inflammatory factor levels in patients with schizophrenia and their relationship with metabolic indicators.Method:115 patients with schizophrenia(case group)and 54 healthy controls(control group)were enrolled.The metabolism-related indicators were collected and the concentration of 9 inflammatory factors[interleukin(IL)-1β,IL-4,IL-6,IL-8,IL-10,IL-12,tumor necrosis factor(TNF-α),interferon(IFN-γ)and chemokines(CCL-2)]were determined.Inflammatory factor levels of the two groups were compared,and Speaman correlation analysis was used to explore the correlation between abnormal inflammation and various metabolics,including Body Mass Index(BMI),abdominal circumference,hip circumference,blood pressure,fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),triglycerides(TG),total cholesterol(CHO),high-density lipoprotein(HDL),Low-Density Lipoprotein(LDL).Results:The levels of IL-1β,IL-4,IL-6,IL-8,TNF-αand CCL-2 in the case group were significantly higher than those in the control group(P<0.05).Serum IL-4 levels in the case group were positively correlated with BMI(r=0.20,P=0.03)and TG(r=0.22,P=0.02)levels;IL-8 levels were negatively correlated with FPG(r=-0.25,P=0.01)and HDL(r=-0.26,P=0.01)levels;IL-6(r=-0.21,P=0.03)and TNF-α(r=-0.29,P<0.01)levels were negatively correlated with HDL levels;CCL-2 levels were positively correlated with BMI(r=0.19,P=0.04),abdominal circumference(r=0.21,P=0.03)and the level of TG(r=0.23,P=0.02).Conclusion:Patients with schizophrenia have elevated levels of various inflammatory factors,and this immune function disorder is related to the metabolic disorder that accompanies the disease.
