摘  要:目的探讨事件相关电位P300对丘脑梗死后认知障碍的诊断价值。方法选取2020年5月至2021年7月于蚌埠市第三人民医院住院治疗的急性丘脑梗死患者30例纳入丘脑梗死组,随访6个月后根据蒙特利尔认知评估量表(Montreal cognitive assessment,MoCA)评分结果将丘脑梗死组分为丘脑梗死后认知障碍组(MoCA评分<26分)和丘脑梗死后非认知障碍组(MoCA评分≥26分),选取同期门诊就诊、无脑血管疾病且认知正常的20名健康者纳入对照组,分析事件相关电位P300对丘脑梗死后认知障碍的预测价值。结果丘脑梗死后认知障碍的发病率为76.7%。丘脑梗死后认知障碍组患者MoCA评分、受教育年限均显著低于丘脑梗死后非认知障碍组和对照组(P<0.05)。丘脑梗死后认知障碍组患者事件相关电位P300潜伏期显著长于丘脑梗死后非认知障碍组和对照组,P300波幅低于丘脑梗死非认知障碍组和对照组(P<0.05);事件相关电位P300潜伏期延长是丘脑梗死后认知障碍的独立危险因素(P<0.05)。事件相关电位P300潜伏期诊断丘脑梗死后认知障碍的曲线下面积为0.853,敏感度为84.2%,特异性为88.9%。结论丘脑梗死后易出现认知障碍,事件相关电位P300潜伏期延长是丘脑梗死后认知障碍的危险因素,事件相关电位P300潜伏期可作为丘脑梗死后认知障碍的预测工具。Objective To explore the value of event-related potential P300 in the diagnosis of cognitive impairment after thalamic infarction.Methods A total of 30 patients with acute thalamic infarction hospitalized in the Third People’s Hospital of Bengbu from May 2020 to July 2021 were included in the thalamic infarction group.After 6 months of follow-up,according to the Montreal cognitive assessment(MoCA)score,the thalamic infarction group was divided into the post-thalamic infarction cognitive impairment group(MoCA score<26)and the post-thalamic infarction non-cognitive impairment group(MoCA score≥26).Twenty healthy people without cerebrovascular dis-ease and with normal cognition were included in control group.The predictive value of event-related potential P300 on cognitive impairment after thalamic infarction was analyzed.Results The incidence of cognitive impairment after thalamic infarction was 76.6%.MoCA score and years of education in post-thalamic infarction cognitive impair-ment group were significantly lower than those in post-thalamic infarction non-cognitive impairment group and con-trol group(P<0.05).The latency of event related potential P300 in post-thalamic infarction cognitive impairment group was significantly longer than that in post-thalamic infarction non-cognitive impairment group and the control group,and the amplitude of P300 was lower than that in post-thalamic infarction non-cognitive impairment group and the control group(P<0.05).Prolonged incubation period of event-related potential P300 was an independent risk factor for cognitive impairment after thalamic infarction(P<0.05).The area under the curve,sensitivity and specificity of event-related pote-ntial P300 for the diagnosis of cognitive impairment after thalamic infarction were 0.853,84.2%and 88.9%respectively.Conclusion Thalamic infarction is prone to cognitive impairment.The prolonged incubation period of event-related potential P300 is a risk factor for cognitive impairment after thalamic infarction,which can be used as a predictiv
