清胰化积方联合PTCD术治疗晚期胰头癌伴梗阻性黄疸回顾性研究

摘　　要：目的观察清胰化积方对晚期胰头癌伴梗阻性黄疸患者行经皮经肝穿刺胆道引流术(PTCD)术后肝功能、感染率及生存期的影响。方法选取复旦大学附属肿瘤医院2013年11月—2019年12月行PTCD的47例晚期胰头癌伴梗阻性黄疸患者的临床资料,将其分为清胰化积组(24例)和对照组(23例)。比较2组患者PTCD术后的肝功能变化、感染率及总生存率。结果PTCD术后第28天,清胰化积组患者的血清总胆红素(TBIL)、直接胆红素(DBIL)、谷丙转氨酶(ALT)、碱性磷酸酶(ALP)和γ-谷氨酰转肽酶(GGT)水平均低于对照组,差异有统计学意义(P<0.05)。2组患者总中位生存期为6.6个月,其中清胰化积组患者中位生存期为6.8个月,对照组中位生存期为6.1个月,2组比较差异有统计学意义(P<0.05)。结论与对照组相比,清胰化积方能促进晚期胰头癌伴梗阻性黄疸患者PTCD术后的肝功能恢复,延长患者生存时间。Objective To observe the effects of Qingyi Huaji formula on liver function,biliary tract infection rate and the influence on the survival time in advanced pancreatic head cancer patients with obstructive jaundices after percutaneous transhepatic cholangial drainage(PTCD).Methods A total of 47 advanced pancreatic head cancer patients with obstructive jaundices were enrolled from November 2013 to December 2019.All of the patients underwent the PTCD surgery.The 47 patients were divided into Qingyi Huaji formula group(24 cases)and control group(23 cases)based on their treatment.The liver function,biliary tract infection rate and overall survival after PTCD were analyzed.Results On the 28th day after the surgery,levels of TBIL,DBIL,TBA,ALP,GGT and ALT in the Qingyi Huaji formula group were lower than those in the control group(P<0.05).The median survival time of all patients was 6.6 months.In the Qingyi Huaji formula group,the median survival time was 6.8 months,which was significantly higher than that in the control group(6.1 months)(P<0.05).Conclusion Qingyi Huaji formula treatment could promote the recovery of liver function,prolong the overall survival time for patients with advanced pancreatic head cancer patients with obstructive jaundices who underwent PTCD.
