扶正解毒汤联合替诺福韦酯治疗慢性乙型病毒性肝炎临床研究

摘　　要：目的:观察扶正解毒汤联合替诺福韦酯治疗慢性乙型病毒性肝炎(简称慢性乙肝)的临床疗效。方法:选取100例慢性乙肝患者,按照随机数字表法分为试验组和对照组各50例。对照组给予替诺福韦酯治疗,试验组在对照组基础上联合扶正解毒汤治疗,2组均治疗3个月。比较2组乙肝病毒表面抗原(HBsAg)、乙肝病毒e抗原(HBeAg)、乙肝病毒脱氧核糖核酸(HBV-DNA)转阴率;比较2组治疗前后肝功能指标[谷丙转氨酶(ALT)、谷草转氨酶(AST)]水平,免疫球蛋白(Ig)A、IgM、IgG水平,以及免疫功能指标[CD3、CD4、自然杀伤细胞(NK细胞)]水平。结果:试验组HBV-DNA、HBsAg、HBeAg转阴率均高于对照组(P<0.05)。治疗后,2组ALT、AST、IgA、IgM、IgG水平均较治疗前降低(P<0.05),试验组ALT、AST、IgA、IgM、IgG水平均低于对照组(P<0.05)。治疗后,2组CD3、CD4、NK细胞水平均较治疗前升高(P<0.05),试验组CD3、CD4、NK细胞水平均高于对照组(P<0.05)。结论:扶正解毒汤联合替诺福韦酯治疗慢性乙肝,能提高患者HBV-DNA、HBsAg、HBeAg转阴率,改善其肝功能和免疫功能。Objective:To observe the clinical effect of Fuzheng Jiedu tang combined with tenofovir disoproxil for chronic viral hepatitis B(chronic hepatitis B).Methods:A total of 100 cases of patients with chronic hepatitis B were selected and divided into the experiment group and the control group according to the random number table method,with 50 cases in each group.The control group was treated with tenofovir disoproxil,and the experiment group was additionally treated with Fuzheng Jiedu tang based on the treatment of the control group.Both groups were treated for three months.The negative conversion rates of hepatitis B virus surface antigen(HBsAg),hepatitis B virus e antigen(HBeAg)and hepatitis B virus deoxyribonucleic acid(HBV-DNA)in the two groups were compared;the levels of the liver function indexes including alanine transaminase(ALT)and aspartate transaminase(AST),the immunoglobulin(Ig)A,IgM,IgG,and the immune function indexes including CD3,CD4and natural killer cell(NK cell)before and after treatment in the two groups were compared.Results:The negative rates of HBV-DNA,HBsAg and HBeAg in the experiment group were higher than those in the control group(P<0.05).After treatment,the levels of ALT,AST,IgA,IgM and IgG in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of ALT,AST,IgA,IgM and IgG in the experiment group were lower than those in the control group(P<0.05).After treatment,the levels of CD3,CD4and NK cell in the two groups were increased when compared with those before treatment(P<0.05),and the levels of CD3,CD4and NK cell in the experiment group were higher than those in the control group(P<0.05).Conclusion:Fuzheng Jiedu tang combined with tenofovir disoproxil for chronic hepatitis B can enhance the negative rates of HBV-DNA,HBsAg and HBeAg,and improve the liver function and immune function.
