首发伴自杀意念抑郁症患者静息态功能磁共振特征及其与临床特征的关系

摘　　要：目的:分析首发伴自杀意念(SI)抑郁症患者静息态功能磁共振(fMRI)特征,并探讨其与患者临床特征的关系。方法:收集2018年10月至2020年10月某院诊治的62例首发抑郁症患者,其中伴自杀意念患者34例(SI组),不伴自杀意念患者28例(NSI组);分别采用汉密尔顿抑郁量表17项(HAMD-17)和Beck自杀意念量表对患者抑郁程度和自杀意念强度进行评估;两组均行fMRI扫描,采用比率低频振幅(ALFF)分析两组脑功能活动差异。结果:SI组HAMD-17评分和Beck自杀意念强度因子均显著高于NSI组(t=7.679,12.384;P<0.001);SI组左侧枕中回/左侧枕下回、右侧枕中回脑区fALFF值均显著高于NSI组(t=3.571,2.186;P<0.05);Pearson相关性分析显示,SI组左侧枕中回/左侧枕下回和右侧枕中回fALFF值均与HAMD-17评分呈显著正相关(r=0.487,0.425;P<0.01),而与Beck自杀意念强度因子均无显著相关性。结论:伴SI抑郁症患者左侧枕中回/左侧枕下回和右侧枕中回脑区存在神经功能活动异常,并与抑郁严重程度密切相关。Objective:To analyze the characteristics of functional magnetic resonance imaging(fMRI)in patients with first-episode depression and suicidal ideation(SI),and to explore their relationship with clinical characteristics of patients.Methods:A total of 62 patients with first-episode depression treated in the hospital were enrolled between October 2018 and October 2020,including 34 cases with SI(SI group)and 28 cases without SI(NSI group).The depression and SI were assessed by 17-item Hamilton Depression Scale(HAMD-17)and Beck Suicide Ideation Scale.Both groups underwent fMRI scan.The differences in brain function activity between the two groups were analyzed by amplitude of low-frequency fluctuation(ALFF).Results:HAMD-17 score and Beck suicidal ideation intensity factor in SI group were significantly higher than those in NSI group(t=7.679,12.384;P<0.001).The fALFF value of left middle occipital gyrus/left inferior occipital gyrus and fALFF value of right middle occipital gyrus in SI group were significantly higher than those in NSI group(t=3.571,2.186;P<0.05).Pearson correlation analysis showed that fALFF values of left middle occipital gyrus/left inferior occipital gyrus and right middle occipital gyrus were significantly positively correlated with HAMD-17 score in SI group(r=0.487,0.425;P<0.01),but not significantly correlated with Beck suicide ideation intensity factor.Conclusion:For patients with depression and SI,there are abnormal neurological activities in left middle occipital gyrus/left inferior occipital gyrus and right middle occipital gyrus,which are closely related to the severity of depression.
