摘　　要：目的 探讨低剂量螺旋CT仿真内镜技术(CTVE)在早期结直肠癌诊断中的筛查价值。方法 回顾性分析2015年1月—2020年7月在张家口市第一医院接受治疗的375例疑似早期结直肠癌患者的临床资料,均采用常规CT和低剂量CTVE对患者进行检查,根据金标准(病理检查结果)分析检查结果,并比较其辐射效果。结果 375例患者经病理检查确诊:恶性315例,良性60例;常规CT检查发现:真恶性290例,真良性46例,误诊14例,漏诊25例。低剂量CTVE结果显示,真恶性312例,真恶性58例,误诊2例,漏诊3例。与常规CT检查比较,低剂量CTVE检查的灵敏度、特异度、恶性预测值、良性预测值及诊断准确率更高,差异具有统计学意义(P<0.05);低剂量CTVE检查的CTDlw、DLP与常规CT检查相比明显更低,差异具有统计学意义(P<0.05)。结论 采用低剂量CTVE对早期结直肠癌患者进行检查,可以有效降低辐射剂量,具有较高的安全性,且敏感性较高,能够在一定程度上提高早期结直肠癌的诊断准确率,具有较高的临床应用价值。Objective To investigate the screening value of low-dose spiral CT virtual endoscopy(CTVE)in early colorectal cancer.Methods The clinical data of 375patients with suspected early-stage colorectal cancer who were treated in Zhangjiakou First hospital from January 2015to July 2020were retrospectively analyzed.Conventional CT and low-dose CTVE were used to examine them.Inspection results)analyze the inspection results of the two groups of inspection methods and compare their radiation effects.Results Pathological examination of 375patients found that malignant and benign patients were 315and 60,respectively;routine CT examination found true malignant,true benign,misdiagnosed and missed diagnosis of 290,46,14,and 25cases;low-dose CTVE results It showed that there were 312,58,2,and 3cases of true malignant,true malignant,misdiagnosed,and missed diagnosis,respectively.Compared with conventional CT examination,the sensitivity,specificity,malignant predictive value,benign predictive value and diagnostic accuracy of low-dose CTVE examination were significantly higher,and the difference was statistically significant(P<0.05).Compared with conventional CT examination,it was significantly lower,and the difference was statistically significant(P<0.05).Conclusion The use of low-dose CTVE in the examination of early colorectal cancer patients can effectively reduce the radiation dose,with high safety,and the detection method has high sensitivity,which can improve the diagnosis of early colorectal cancer to a certain extent.Accuracy,high clinical application value.
