摘　　要：目的 探讨肝硬化合并门静脉血栓患者手术前后血浆总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)和载脂蛋白A1(apo-A1)水平变化及其临床意义。方法 选取2017年7月至2019年7月在广元市中心医院进行治疗的125例肝硬化合并门静脉血栓患者为研究对象,将其纳入血栓组,另选取同期在医院进行治疗的113例肝硬化无合并静脉血栓患者作为对照组,比较两组TC、HDL-C、apo-A1水平,并记录血栓组患者在治疗前后以上指标的变化情况。结果 血栓组TC、HDL-C、apo-A1水平明显低于对照组,且随着疾病分期发展,TC、HDL-C、apo-A1水平逐渐降低(P<0.05);治疗后肝硬化合并门静脉血栓患者TC、HDL-C、apo-A1水平明显高于治疗前(P<0.05);存活组TC、HDLC、apo-A1均显著高于病死组(P<0.05)。结论 肝硬化合并门静脉血栓患者在治疗后TC、HDL-C、apo-A1均有所升高,且在预后评估中具有指导意义。Objective To investigate the changes and clinical significance of plasma total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C)and apolipoprotein A1(apo-A1)levels in patients with liver cirrhosis and portal vein thrombosis before and after operation.Methods 125patients with liver cirrhosis and portal vein thrombosis who were treated in the hospital from July 2017to July 2019were selected and included in the thrombosis group.Meanwhile,113patients with liver cirrhosis and without venous thrombosis were selected as control group.The levels of TC,HDL-C and apo-A1were compared between the two groups,and changes of the above indicators in the thrombosis group before and after treatment were recorded.Results The levels of TC,HDL-C and apo-A1in thrombosis group were significantly lower than those in the control group.With the development of disease,the levels of above indicators decreased(P<0.05).After treatment,the levels of TC,HDL-C and apo-A1in thrombosis group were significantly increased(P<0.05).Conclusion TC,HDL-C and apo-A1are elevated in patients with liver cirrhosis and portal vein thrombosis after treatment.TC,HDL-C and apo-A1can be used to diagnose liver cirrhosis with portal vein thrombosis.
