摘 要:目的探讨单床位采集时间对^(68)Ga-1,4,7,10-四氮杂环十二烷-1,4,7,10-四乙酸-1-萘丙氨酸3-奥曲肽(^(68)Ga-DOTA-NOC)正电子发射计算机体层摄影术(PET)图像质量的影响和临床分析。方法选择行^(68)Ga DOTA-NOC PET检查的神经内分泌肿瘤患者23例(43处病灶),其中男性9例,女性14例;年龄23~73岁,平均年龄50.35岁。在标准体模中4个直径为10 mm、13 mm、17 mm、22 mm球体注射比例为4∶1的^(68)Ga-DOTA-NOC。用变异系数(CV)值、信噪比(SNR)、对比度、肝脏标准摄取平均值(SUV_(mean))、肝脏SUV_(SD)、肝脏CV_(liver)值和病灶SUV_(max)等方法来评估图像质量。结果单床位采集时间增加时体模CV值走向为逐渐下降,对比度走向较为稳定,SNR走向为上升的状态(体模22 mm时CV从23.64%降至7.58%;对比度浮动范围为5.28~6.29;SNR从18.48升至56.52。体模17 mm时CV从19.94%降至6.62%;对比度浮动范围为5.30~6.03;SNR从25.19升至63.04。体模13 mm时CV从27.49%降至5.02%;对比度浮动范围为4.67~4.74;SNR从20.58上升54.50。体模10 mm时CV从14.48%降至7.54%;对比度浮动范围为3.63~3.25;SNR从18.16升至49.79)。当单床位采集时间为3 min时PET图像病灶清晰可见,图像质量优秀。肝脏SUV_(SD)和CV_(liver)值两者对应的单床位采集时间差异有统计学意义(F=5.003、3.048,P<0.05),肝脏SUV_(mean)得分差异无统计学意义(P>0.05),病灶SUV_(max)整体得分差异无统计学意义(F=0.030,P=0.9983)。结论单床位采集时间对^(68)GaDOTA-NOC PET图像质量和SUV值有着较大的影响,通过单床位采集时间的增加即可降低噪声改善图像质量。单床位采集为3 min^(68)Ga-DOTA-NOC PET低剂量扫描可满足临床诊断。Objective To explore the influence and clinical analysis of single bed acquisition time on ^(68)Ga-1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid-1-Nal3-octreotide(^(68)Ga-DOTA-NOC)PET image quality.Method A total of23 neuroendocrine tumor patients(43 lesions)performed ^(68)Ga DOTA-NOC PET,which included 9 males and 14 females,aged 23-73 years old with mean age of 50.35 years old.In standard phantom,4 spheres with diameters of 10 mm,13 mm,17 mm and 22 mm were injected with ^(68)Ga-DOTA-NOC at the ratio of 4∶1.The image quality was assessed by coefficient of variation(CV),signal-to-noise ratio(SNR),contrast,liver SUV_(mean),liver SUV_(SD),liver CV_(liver)and lesion SUV_(max).Results The phantom CV gradually decreased,contrast was relatively stable and SNR increased as single bed acquisition time increased(At 22 mm phantom,CV decreased from 23.64%to 7.58%,contrast fluctuation range was 5.28-6.29,SNR increased from 18.48 to 56.52.At 17 mm phantom,CV decreased from 19.94%to 6.62%,contrast fluctuation range was 5.30-6.03,SNR increased from 25.19 to 63.04.At 13 mm phantom,CV decreased from 27.49%to 5.02%,contrast fluctuation range was 4.67-4.74,SNR increased from 20.58 to 54.50.At 10 mm phantom,CV decreased from 14.48%to 7.54%,contrast fluctuation range was 3.63-3.25,SNR increased from 18.16 to 49.79).When the single bed acquisition time reached3 minutes,PET image of lesions were clear and image quality was excellent.There was statistically significant difference in single bed acquisition time between liver SUV_(SD)and CV_(liver)(F=5.003,3.048,P<0.05),and there were no significant difference in liver SUV_(mean)(P>0.05)as well as overall score of lesion SUV_(max)(F=0.030,P=0.9983).Conclusion It is demonstrated that acquisition time of ^(68)Ga-DOTA-NOC PET showed great influence on image quality and SUV value,and increasing acquisition time of single bed could reduce noise and improve image quality.The single bed acquisition for 3-minute ^(68)Ga-DOTA-NOC PET low-dose scanning could meet clinical diagnosis.
