棒柄花叶清肝胶囊联合恩卡替韦治疗HBeAg阴性慢性乙型肝炎的临床疗效

摘　　要：目的:评价棒柄花叶清肝胶囊联合恩卡替韦分散片治疗HBeAg阴性慢性乙型肝炎的有效性及其安全性。方法:将符合纳入标准的90例患者随机分成两组,各45例。西医治疗组患者口服恩替卡韦分散片0.5 mg/次,每天1次;棒柄花叶治疗组患者在此基础上加用棒柄花叶清肝胶囊4粒/次,每日3次,两组患者均治疗24周,观察两组患者治疗后的临床疗效、生化应答情况(ALT)、HBV DNA转阴率、肝纤维指标变化情况(HA、PCⅢ、Ⅳ-C、LN),并记录安全性相关指标。结果:治疗24周后,棒柄花叶治疗组有效率为91.1%,高于西医治疗组(77.8%,P<0.05);棒柄花叶治疗组患者临床症状积分、ALT与西医治疗组患者比较差异有统计学意义(P<0.05);棒柄花叶治疗组HBV DNA阴转率为73.3%,西医治疗组阴转率为62.2%,治疗期间未出现不良反应事件。结论:棒柄花叶清肝胶囊联合恩卡替韦分散片治疗HBeAg阴性慢性乙型肝炎的临床疗效优于单独使用恩卡替韦分散片治疗,且可改善患者症状体征,降低ALT,提高HBV DNA阴转率,且具有一定的抗肝纤维化作用,安全性良好。Objective:To evaluate the efficacy and safety of Bangbing Huaye Qinggan capsule combined with entecavir dispersible tablets in the treatment of HBeAg negative chronic hepatitis B.Methods:A total of 90 patients who met the diagnostic criteria were randomly divided into western medicine treatment group and Bangbing Huaye treatment group,45 cases in each group.The western medicine treatment group was treated with entecavir dispersible tablets orally,once a day,0.5mg/time;the Bangbing Huaye treatment group was treated with Bangbing Huaye Qinggan capsule additionally,4 capsules once a day,3 times a day.The observation period of the two groups was 24 weeks After treatment,the clinical efficacy,biochemical response(ALT),HBV DNA negative rate,liver fiber index changes(HA,PC-Ⅲ,IV-C,LN),and safety related indicators were recorded.Results:After treatment,the effective rate of the treatment group was 91.1%,which was higher than 77.8%of the western medicine treatment group,the difference was statistically significant(P<0.05),the symptom score of the treatment group was significantly different from that of the western medicine treatment group(P<0.05),the ALT of the treatment group was significantly lower than that of the western medicine treatment group,the difference was statistically significant(P<0.05).The negative conversion rate of HBV DNA was 73.3%in the treatment group and 62.2%in the western medicine group,and no adverse reactions occurred during the treatment.Conclusion:The clinical efficacy of Bangbing Huaye Qinggan capsule combined with entecavir dispersible tablets in the treatment of HBeAg negative chronic hepatitis B is better than that of entecavir dispersible tablets alone,and it can improve the symptoms and signs of HBeAg negative patients with chronic hepatitis B,reduce the levels of transaminase and liver fiber,improve the negative conversion rate of HBV DNA,and has a certain anti-fibrosis effect with good safety.
