摘　　要：目的:探讨血清生长分化因子-15(GDF-15)、脂蛋白相关磷脂酶A2(LP-PLA2)、缺血修饰蛋白(IMA)在急性缺血性脑卒中(AIS)患者中的表达,并分析其与NIHSS评分的相关性。方法:收集2021年1-12月扬州市中医院收治的120例AIS患者作为AIS组,根据美国国立卫生院卒中量表(NIHSS评分)将AIS患者分为轻度组48例(NIHSS评分0~5分)、中度组40例(NIHSS评分6~12分)及重度组32例(NIHSS评分>12分)。同期收集体检中心健康体检者100例作为对照组。比较各组间血清GDF-15、LP-PLA2、IMA表达差异,并利用Spearman相关性分析其与NIHSS评分相关性。结果:AIS组患者血清GDF-15、LP-PLA2、IMA水平明显高于对照组(P<0.05);三者联合检测的曲线下面积为0.883,敏感度和特异度为93.1%、90.7%。重度组患者GDF-15、LP-PLA2、IMA水平均明显高于中度组,中度组均明显高于轻度组(P<0.05)。Spearman相关性分析显示,GDF-15、LP-PLA2、IMA与NIHSS评分呈正相关(P<0.05)。结论:AIS患者GDF-15、LP-PLA2、IMA水平明显较高,且与NIHSS评分密切相关,联合检测有助于早期诊断及病情评估。Objective:To investigate the expressions of serum growth differentiation factor-15(GDF-15),lipoprotein-associated phospholipase A2(LP-PLA2)and ischemia modified protein(IMA)in patients with acute ischemic stroke(AIS),and to analyze their correlation with NIHSS score.Method:A total of 120 patients with AIS admitted to Yangzhou Hospital of Traditional Chinese Medicine from January to December 2021 were enrolled as AIS group.According to the national institutes of health stroke scale(NIHSS score),AIS patients were divided into 48 cases in mild group(NIHSS was 0-5 points),40 cases in moderate group(NIHSS score was 6-12 points),and 32 cases in severe group(NIHSS score was above 12 points).During the same period,100 healthy subjects in the physical examination center were collected as the control group.The expressions of GDF-15,LP-PLA2 and IMA in serum were compared among all groups,and the correlation between them and NIHSS score was analyzed by Spearman correlation.Result:The serum levels of GDF-15,LP-PLA2 and IMA in AIS group were significantly higher than those in control group(P<0.05).The area under the curve was 0.883,and the sensitivity and specificity were 93.1%and 90.7%.The levels of GDF-15,LP-PLA2 and IMA in severe group were significantly higher than those in moderate group,and those in moderate group were significantly higher than those in mild group(P<0.05).Spearman correlation analysis showed that GDF-15,LP-PLA2,IMA were positively correlated with NIHSS score(P<0.05).Conclusion:The levels of GDF-15,LP-PLA2 and IMA are significantly increased in patients with AIS,and are closely correlated with NIHSS score.Combined detection is helpful for early diagnosis and disease evaluation.
