摘  要:目的探讨粪菌移植治疗对腹泻型肠易激综合征(IBS-D)模型大鼠脑-肠轴中5-羟色胺(5-HT)信号通路的影响。方法选取45只SPF级健康SD雄性大鼠,按照随机数字表法分为正常组、模型组及粪菌移植组,每组15只,采用番泻叶灌胃法联合束缚应激构建IBS-D动物模型,除正常组外,其他组均进行造模。粪菌移植组进行粪菌液灌肠处理,每次灌肠体积2 mL,连续灌肠治疗10 d。观察大鼠的生活状况和粪便含水量,酶联免疫吸附试验(ELISA)检测大鼠脑组织及血清中5-HT和P物质(SP)的表达水平,免疫组化法检测大鼠结肠及脑组织5-HT转运体(SERT)蛋白表达水平,Western blot法检测大鼠结肠组织和脑组织中SERT、色氨酸羟化酶1(THP1)、5-HT受体3(5-HT3R)、5-HT受体4(5-HT4R)蛋白表达水平。结果与正常组相比,模型组大鼠在造模后,出现稀便样腹泻、精神状况萎靡、饮水量增加、进食量减少,以及烦躁不安等症状,脑组织和血清5-HT和SP表达水平明显升高,结肠组织和脑组织SERT、5-HT4R蛋白表达水平明显降低,THP1和5-HT3R蛋白表达水平明显升高,差异均有统计学意义(P<0.05);与模型组相比,粪菌移植组在治疗后,精神状况明显改善,腹泻状况明显好转,粪便含水量明显降低,脑组织和血清5-HT和SP表达水平明显降低,结肠组织和脑组织SERT、5-HT4R蛋白表达水平明显升高,THP1和5-HT3R蛋白表达水平明显降低,差异均有统计学意义(P<0.05)。结论粪菌移植能够明显改善IBS-D的症状,其机制可能与调节脑-肠轴5-HT信号通路有关。Objective To explore the effect of fecal bacteria transplantation on the serotonin(5-HT)signal pathway in the brain-gut axis of diarrhea-type irritable bowel syndrome(IBS-D)model rats.Methods Forty-five SPF-grade healthy SD male rats were randomly divided into normal group,model group and fecal bacteria transplantation group,with 15 rats in each group.The IBS-D animal model was establish by nsing senna leaf gavage combined with restraint stress,and the model was established in all the groups except the normal group.Fecal bacteria transplantation group was treated with fecal bacteria liquid enema,volume of 2 mL each time,continuous enema treatment for 10 days.The living conditions and fecal water content of rats were observed.The expression levels of 5-HT and SP in brain tissue and serum of rats were detected by ELISA,and the protein expression levels of 5-HT transporter(SERT)in colon and brain of rats were detected by immunohistochemistry.Western blot was used to detect the protein expression levels of SERT,tryptophan hydroxylase 1(THP1),5-HT receptor 3(5-HT3 R)and 5-HT receptor 4(5-HT4 R)in colon and brain tissues of rats.Results Compared with the normal group,the rats in the model group showed symptoms such as watery diarrhea,listless mental state,increased water intake,decreased food intake,and restlessness.The expression levels of 5-HT and SP in brain tissue and serum increased significantly,and the expression levels of SERT and 5-HT4 R in colon tissue and brain tissue decreased significantly,the protein expression levels of THP1 and 5-HT3 R increased significantly,and the differences were statistically significant(P<0.05).Compared with the model group,the mental status,diarrhea,fecal water content of the fecal bacteria transplantation group improved significantly.The expression levels of 5-HT and SP in brain tissue and serum,SERT and 5-HT4 R in colon tissue and brain tissue decreased significantly,as well as THP1 and 5-HT3 R(P<0.05).Conclusion Fecal bacteria transplantation could significantly improve the sy
