摘　　要：目的初步探讨注水肠镜联合锥形帽逆行阑尾炎治疗术治疗急性非复杂性阑尾炎的临床效果。方法收集2019年6月至2021年8月在我院住院的急性非复杂性阑尾炎患者40例,随机分为观察组(n=20例)和对照组(n=20例),观察组采用注水肠镜联合锥形帽逆行阑尾炎治疗术,对照组采用普通肠镜联合普通透明帽逆行性阑尾炎治疗术,比较两组手术情况(进镜时间、插管时间、手术完成时间)、患者腹痛情况及术后治疗效果。结果观察组患者进镜时间多于对照组(P<0.05),而插管时间观察组费时更少,差异有统计学意义(P<0.05)。术前、术后两组患者腹痛VAS评分比较差异无统计学意义(P>0.05),而术中对照组患者腹痛更明显(P<0.05)。两组治疗效果对比较差异无统计学意义(P>0.05)。结论注水肠镜联合锥形帽ERAT术可媲美传统ERAT术的治疗效果,但前者可增加插管成功率,同时缓解患者术中腹痛,更适用于不耐受麻醉治疗患者。Objective To explore clinical effects of water injection enteroscopy combined with conical cap retrograde appendicitis to treat acute uncomplicated appendicitis.Methods From June 2019 to August 2021,forty patients with acute uncomplicated appendicitis hospitalized in our hospital were randomly divided into observation group(OG,n=20)and control group(CG,n=20).OG was treated with water injection enteroscopy combined with conical cap retrograde appendicitis,and CG was treated with ordinary enteroscopy combined with ordinary transparent cap retrograde appendicitis.Operation conditions(endoscopy time,intubation time,operation completion time),abdominal pain and postoperative treatment effect were compared.Results Endoscopy time in OG was more than that in CG(P<0.05),while intubation time in OG was less time-consuming(P<0.05).There was no significant difference in VAS score of abdominal pain before and after operation(P>0.05),but abdominal pain in CG was more obvious during operation(P<0.05).There was no significant difference in treatment effect(P>0.05).Conclusion Therapeutic effect of water injection enteroscopy combined with conical cap erat would be comparable to that of traditional erat,but the former could increase success rate of intubation and relieve intraoperative abdominal pain,which would be more suitable for patients who could not tolerate anesthesia.
