术前中性粒细胞与淋巴细胞比值在壶腹癌患者预后中的预测价值

摘　　要：目的 分析术前中性粒细胞与淋巴细胞比值(NLR)在壶腹癌患者预后中的预测价值。方法 选择2010年1月至2012年5月我院收治的135例壶腹癌患者为研究对象,收集患者的临床信息资料,检测术前中性粒细胞绝对数(ANC)、淋巴细胞绝对计数(ALC),并计算NLR。采用单因素分析和多因素Cox回归分析法分析术前NLR在术后壶腹癌患者预后中的预测价值。结果 单因素分析结果显示,不同性别、年龄、T分期、淋巴结受累情况、术后ECOG评分分级、术后辅助治疗、术前ANC的患者术后3、5、10年总生存期(OS)比较,差异无统计学意义(P>0.05);手术切缘状态、术前ALC和NLR是影响壶腹癌患者预后的相关影响因素(P<0.05)。多因素Cox回归分析结果显示,手术切缘、术前NLR是影响壶腹癌患者预后的独立危险因素(P<0.05)。不同NLR水平患者的术后ECOG评分分级和术前ANC比较,差异具有统计学意义(P<0.05)。结论 手术切缘、术前NLR是影响壶腹癌患者预后的独立危险因素。通过NLR预测患者的预后情况,有助于未来壶腹癌患者的临床监测和管理工作。Objective To analyze the predictive value of preoperative neutrophil-to-lymphocyte ratio(NLR)in the prognosis of patients with ampullary carcinoma.Methods A total of 135 patients with ampullary carcinoma admitted in our hospital from January 2010 to May 2012 were selected as the study objects.The clinical information data of patients was collected,the preoperative absolute neutrophil count(ANC)and the absolute lymphocyte count(ALC)were detected,and the NLR was calculated.Univariate analysis and multivariate Cox regression analysis method were used to analyze the predictive value of preoperative NLR in the prognosis of patients with ampullary carcinoma.Results Univariate analysis results showed that there were no significant differences in overall survival(OS)of patients with different gender,age,T stage,lymph node involvement,postoperative ECOG score rating,postoperative adjuvant treatment and preoperative ANC at 3,5,and 10 years after operation(P>0.05);the surgical margin status,preoperative ALC and NLR were the related factors affecting the prognosis of patients with ampullary carcinoma(P<0.05).Multivariate Cox regression analysis results showed that surgical margin and preoperative NLR were independent risk factors affecting the prognosis of patients with ampullary carcinoma(P<0.05).There were significant differences in the postoperative ECOG score rating and preoperative ANC of patients with different NLR level(P<0.05).Conclusion The surgical margin and preoperative NLR are independent risk factors affecting the prognosis of patients with ampullary carcinoma.The prediction of patients prognosis by NLR is helpful for clinical monitoring and management of ampullary carcinoma patients in the future.
