鼻内镜下低温等离子消融术治疗鼻腔血管瘤的临床效果及安全性

摘　　要：目的 探究鼻内镜下低温等离子消融术治疗鼻腔血管瘤的临床效果及安全性。方法 选取2018年5月至2021年5月我院耳鼻喉科收治的200例鼻腔血管瘤患者作为研究对象,将其随机分为对照组和观察组,各100例。对照组行常规鼻内镜下切除术,观察组行鼻内镜下低温等离子消融术。比较两组的临床效果。结果 观察组的手术耗时、术后鼻腔黏膜上皮化时间及住院时间均短于对照组,术中出血量少于对照组,差异具有统计学意义(P<0.05)。观察组的治疗总有效率为96.00%,显著高于对照组的84.00%,差异具有统计学意义(P<0.05)。观察组的并发症总发生率为3.00%,显著低于对照组的13.00%,差异具有统计学意义(P<0.05)。术前,两组的C反应蛋白(CRP)、白细胞介素-6(IL-6)水平比较,差异无统计学意义(P>0.05);术后24 h,两组的CRP、IL-6水平均高于术前,差异具有统计学意义(P<0.05);术后24 h,观察组的CRP、IL-6水平均低于对照组,差异具有统计学意义(P<0.05)。结论 鼻内镜下低温等离子消融术治疗鼻腔血管瘤的临床效果显著,可减少术中出血量,调节炎性因子水平,且安全性高。Objective To investigate the clinical effect and safety of low temperature plasmaablation under nasal endoscopic in the treatment of nasal hemangioma.Methods A total of 200 patients with nasal hemangioma admitted in the otolaryngology department of our hospital from May 2018 to May 2021 were selected as the research objects,and the patients were randomly divided into control group and observation group,with 100 cases in each group.The control group received conventional resection under nasal endoscopic,and the observation group received low temperature plasmaablation under nasal endoscopic.The clinical effects of the two groups were compared.Results The operation time,postoperative nasal mucosa epithelialization time and hospital stay in the observation group were shorter than those in the control group,the intraoperative blood loss was less than that in the control group,and the differences were statistically significant(P<0.05).The total effective rate of treatment in the observation group was 96.00%,which was significantly higher than 84.00%in the control group,and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was 3.00%,which was significantly lower than 13.00%in the control group,and the difference was statistically significant(P<0.05).Before operation,there were no significant differences in C-reaction protein(CRP)and interleukin-6(IL-6)levels between the two groups(P>0.05);at 24 h after operation,the levels of CRP and IL-6 in the two groups were higher than those before treatment,and the differences were statistically significant(P<0.05);at 24 h after operation,the levels of CRP and IL-6 in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Low temperature plasmaablation under nasal endoscopic in the treatment of patients with nasal hemangioma has a significant clinical effect,it can reduce the intraoperative blood loss,regulate the levels of inflammatory f
