无托槽隐形矫治器减数治疗双牙弓前突的效果及对患者面部美学的影响

摘　　要：目的 研究无托槽隐形矫治器减数治疗双牙弓前突的效果及对患者面部美学的影响。方法 将62例双牙弓前突患者根据治疗方法的不同分为传统组和隐形组,各31例。传统组使用直丝弓固定矫治器进行治疗,隐形组使用无托槽隐形矫治器进行治疗。比较两组的治疗效果、治疗参数、牙周组织健康状况指标和面部美学指标。结果 两组的治疗效果比较,差异无统计学意义(P>0.05)。隐形组的总体治疗时间长于传统组,复诊次数少于传统组(P<0.05)。治疗后,传统组的牙龈指数(GI)、菌斑指数(PLI)均高于治疗前及隐形组(P<0.05)。治疗后,两组的下中切牙长轴延长线与眶耳平面的夹角(FMIA)、上下中切牙角(U1-L1)增大,眶耳平面-下颌平面角(FMA)减小,且隐形组优于传统组(P<0.05)。结论 与固定矫治器相比,无托槽隐形矫治器减数治疗双牙弓前突的效果确切,能够明显改善患者的面部美学指标,且对牙周组织影响较小,但是治疗时间较长。Objective To study the effect of bracket-free invisible appliance and reduction in the treatment of double arch protrusion and its influence on the patients'facial aesthetics.Methods A total of 62 patients with double arch protrusion were divided into traditional group and invisible group according to different treatment methods,with 31 cases in each group.The traditional group was treated with straight wire arch fixed appliance,and the invisible group was treated with bracket-free invisible appliance.The treatment effect,treatment parameters,periodontal tissue health state indexes and facial aesthetics indexes were compared between the two groups.Results There was no significant difference in treatment effect between the two groups(P>0.05).The total treatment time of the invisible group was longer than that of the traditional group,and the number of follow-up visits was less than that of the traditional group(P<0.05).After treatment,the gingival index(GI)and plaque index(PLI)of the traditional group were higher than those before treatment and the invisible group(P<0.05).After treatment,the angle between the long axis extension of the lower central incisors and the orbital auricular plane(FMIA)and the angle of the upper and lower central incisors(U1-L1)increased in the two groups,and the Frankfort-mandibular plane angle(FMA)decreased,and those in the invisible group were better than the traditional group(P<0.05).Conclusion Compared with the fixed appliance,the bracket-free invisible appliance and reduction in the treatment of double arch protrusion has a definite effect,which can significantly improve the facial aesthetics indexes of patients,and has less impact on periodontal tissue,but has longer treatment time.
