保乳联合前哨淋巴结活检术在老年乳腺癌治疗中的效果评价

摘　　要：目的 评价保乳联合前哨淋巴结活检术(SLNB)对老年乳腺癌的治疗效果。方法 98例老年乳腺癌患者,随机分为治疗组和基础组,各49例。治疗组行保乳联合SLNB治疗,基础组行基础切除术治疗。比较两组患者手术指标(手术时间、拔管时间、出血量、引流量)、并发症发生情况及生活质量评分。结果 治疗组手术时间(61.40±9.07)min、拔管时间(5.79±1.03)d短于基础组的(158.72±9.44)min、(31.92±6.18)d,出血量(69.83±7.03)ml、引流量(105.71±11.32)ml少于基础组的(95.99±9.64)、(204.78±31.95)ml,差异具有统计学意义(P<0.05)。治疗组并发症发生率8.16%低于基础组的26.53%,差异具有统计学意义(P<0.05)。术前,两组精力、健康、心理、角色评分比较差异无统计学意义(P>0.05);术后,治疗组精力、健康、心理、角色评分分别为(68.43±4.90)、(71.09±4.72)、(69.55±4.03)、(67.49±4.85)分,均高于基础组的(59.23±6.14)、(63.95±6.08)、(55.29±6.94)、(54.37±5.91)分,差异具有统计学意义(P<0.05)。结论 保乳联合SLNB治疗老年乳腺癌可明显改善患者生活质量,缩短手术时间和拔管时间,且出血量、引流量及并发症较少,值得临床推广。Objective To evaluate the effect of breast conserving surgery combined with sentinel lymph node biopsy in the treatment of elderly breast cancer.Methods A total of 98 elderly patients with breast cancer were randomly divided into treatment group and basic group,with 49 cases in each group.The treatment group received breast conserving surgery combined with SLNB,and the basic group received basic resection.The surgical indicators(operation time,extubation time,blood loss,drainage volume),complications and quality of life score were compared between the two groups.Results The operation time(61.40±9.07)min and extubation time(5.79±1.03)d in the treatment group were shorter than(158.72±9.44)min and(31.92±6.18)d in the basic group;the blood loss(69.83±7.03)ml and the drainage volume(105.71±11.32)ml in the treatment group were less than(95.99±9.64)and(204.78±31.95)ml in the basic group;all the differences were statistically significant(P<0.05).The complication rate in the treatment group was 8.16%,which was lower than 26.53%in the basic group,and the difference was statistically significant(P<0.05).Before surgery,there was no statistically significant difference in energy,health,psychology and role scores between the two groups(P>0.05).After surgery,the scores of energy,health,psychology and role in the treatment group were(68.43±4.90),(71.09±4.72),(69.55±4.03)and(67.49±4.85)points,which were higher than(59.23±6.14),(63.95±6.08),(55.29±6.94)and(54.37±5.91)points in the basic group,and the differences were statistically significant(P<0.05).Conclusion Breast conserving surgery combined with SLNB can obviously improve the quality of life of patients,shorten the operation time and extubation time,and the blood loss,drainage volume and complications are less,which is worthy of clinical promotion.
