口腔正畸患者颞下颌关节骨关节病的临床分析

摘　　要：目的 研究口腔正畸患者出现颞下颌关节骨关节病的临床特征。方法 204例口腔正畸患者,统计颞下颌关节骨关节病的发生率,比较不同年龄阶段、性别、安氏分类患者的颞下颌关节骨关节病发生率。结果 204例患者中发生颞下颌关节骨关节病29例(14.22%)。不同年龄阶段患者的颞下颌关节骨关节病发生率比较差异无统计学意义(P>0.05)。女性患者的颞下颌关节骨关节病发生率19.64%(22/112)显著高于男性的7.61%(7/92),差异具有统计学意义(P<0.05)。不同安氏分类患者的颞下颌关节骨关节病发生率比较,差异具有统计学意义(P<0.05);安氏Ⅱ类患者的颞下颌关节骨关节病发生率22.83%(21/92)显著高于安氏Ⅰ类的8.33%(5/60)和安氏Ⅲ类的5.77%(3/52),差异具有统计学意义(P<0.05)。结论 口腔正畸患者较易出现颞下颌关节骨关节病,在任一年龄阶段中都可能发病,且与性别、安氏分类间紧密相关,女性、安氏Ⅱ类更易发病;为此,临床中需要对此类患者给予更多的关注,并进行更具针对性的治疗。Objective To study the clinical features of temporomandibular joint osteoarthristis in orthodontic patients.Methods 204 cases of orthodontic patients,the incidence of temporomandibular joint osteoarthritis was counted,and the incidence of temporomandibular joint osteoarthritis in patients with different age,gender and Angle's classification was compared.Results Among the 204 patients,29 patients(14.22%)developed temporomandibular joint osteoarthristis.There was no statistically significant difference in the incidence of temporomandibular joint osteoarthritis among patients of different ages(P>0.05).The incidence of temporomandibular joint osteoarthritis in female patients was 19.64%(22/112),which was significantly higher than 7.61%(7/92)in males,and the difference was statistically significant(P<0.05).The incidence rate of temporomandibular joint osteoarthritis in patients with different Angle's classification was compared,and the difference was statistically significant(P<0.05).The incidence rate of temporomandibular joint osteoarthritis in patients with Angel's classificationⅡwas 22.83%(21/92),which was significantly higher than 8.33%(5/60)of Angel's classificationⅠ,and5.77%(3/52)of Angel's classificationⅢ.All the differences were statistically significant(P<0.05).Conclusion Orthodontic patients are more likely to develop temporomandibular joint osteoarthristis,which can occur at any age stage,and is closely related to gender and Angle's classification.Females and Angel's classificationⅡare more likely to develop disease;therefore,it is necessary to pay more attention to these patients and carry out more targeted treatment.
