超声异常合并脐血风疹IgM阳性胎儿临床特点总结

摘　　要：目的探究超声异常合并脐血风疹IgM阳性胎儿临床特点。方法回顾性分析2019年1月1日至2019年12月31日在广东省妇幼保健院产前诊断中心因“胎儿超声异常”行脐静脉穿刺术,脐血RV⁃IgM检查结果阳性且排除染色体、微阵列、巨细胞结果阳性胎儿的资料。结果共纳入17例脐血RV⁃IgM检查结果阳性胎儿资料。17例脐血RV⁃IgM阳性胎儿中,羊水风疹PCR阳性12例(70.6%),脐血风疹PCR阳性6例(35.3%)。脐血风疹PCR阳性的6例胎儿羊水风疹PCR均为阳性。超声异常且合并脐血RV⁃IgM(+)的孕妇中,在孕期发现超声异常数目大于等于2个时,羊水RV⁃PCR阳性率为91.7%;孕期发现超声异常数目仅1个时,羊水RV⁃PCR阳性率为20%。结论超声异常且合并脐血RV⁃IgM(+)的孕妇中,在孕期发现超声异常数目≥2个时,羊水RV⁃PCR阳性率远远高于孕期发现超声异常数目为1个时。Objective To explore the clinical characteristics of umbilical cord blood rubella IgM positive fetus with abnormal ultrasound results.Methods A retrospective analysis was performed on the data of fetuses with abnormal ultrasound results combined with positive rubella IgM results in prenatal Diagnosis Center of Maternal and Child Health Hospital of Guangdong Province from January 1,2019 to December 31,2019,and the fetuses with the chromosomal abnormality or fetuses who were infected with cytomegalovirus were excluded.Results A total of 17 fetuses were included in the study.Among the 17 umbilical cord blood RV⁃IgM positive fetuses,amniotic fluid rubella PCR was positive in 12 cases(70.6%)and cord blood rubella PCR positive in 6 cases(35.3%).6 fetuses with positive cord blood rubella PCR were all positive for PCR in amniotic fluid rubella.In pregnant women with abnormal ultrasound and positive cord blood RV⁃IgM results,when the number of ultrasound abnormalities was more than or equal to 2 during pregnancy,the positive rate of amniotic fluid RV⁃PCR was 91.7%.When only one ultrasound abnormality was found during pregnancy,the positive rate of amniotic fluid RV⁃PCR was 20%.Conclusions In the pregnant women with abnormal ultrasound and positive cord blood RV⁃IgM results,when the number of ultrasound abnormalities was more than or equal to 2,the positive rate of RV⁃PCR in amniotic fluid was much higher than that of only one during pregnancy.
