摘 要:目的探讨Brain Time Stack图像融合技术在80 kV管电压下对头部灌注CT图像质量改善的应用价值。方法回顾性分析2021年7—9月在北京医院放射科行头部灌注扫描的31例患者资料。根据头部CT灌注扫描获得的时间-密度曲线,选择动脉峰值期、峰值前一期及峰值后一期,共3个时相的0.5 mm图像,进行Brain Time Stack后处理,并将图像数据分为四组:A组(动脉期)、B组(动脉期结合前一期相)、C组(动脉期结合后一期相)、D组(动脉期结合前一期及后一期)。通过t检验比较4组图像的额叶白质、灰质、侧脑室、延髓、小脑的CT值、标准差值(Standard Deviation,SD)、信噪比(Signal to Noise Ratio,SNR)及对比噪声比(Contrast to Noise Ratio,CNR)的统计学差异。此外,由2名医师分别对4组图像的图像质量进行主观评分,并通过Mann-Whitney U检验对比其统计学差异。结果4组图像的额叶白质、灰质、侧脑室、延髓、小脑的SD值、SNR及CNR均存在统计学差异(P<0.001),其中D组图像的噪声最低、SNR及CNR最高。4组图像之间CT值无统计学差异(P>0.05)。此外,4组图像的主观评分存在统计学差异,D组明显优于A、B、C组(P<0.05)。两医师间主观评分一致性较好,Kappa值为0.817(P<0.001)。结论Brain Time Stack图像融合技术可有效降低降低图像噪声,明显改善图像质量。Objective To explore the application of Brain Time Stack image fusion technology in improving the image quality of brain CT perfusion with 80 kV tube voltage.Methods Data of 31 patients who underwent brain perfusion in the Department of Radiology of Beijing hospital from July to September 2021 were analyzed retrospectively.According to the time density curve obtained by brain CT perfusion,0.5 mm images of three phases were selected for brain time stack post-processing,and the image data were divided into four groups:group A(arterial phase),group B(arterial phase combined with pre-arterial phase),group C(arterial phase combined with post-arterial phase),and group D(arterial phase combined with pre-arterial phase and post-arterial phase).The CT value,standard deviation(SD),signal to noise ratio(SNR)and contrast to noise ratio(CNR)of white matter and gray matter of frontal lobe,lateral ventricle,medulla oblongata and cerebellum of the four groups were compared by t-test.In addition,two doctors scored on the image quality of the four groups,and the statistical differences were compared by Mann-Whitney U test.Results The SD,SNR and CNR values of frontal white matter,gray matter,lateral ventricle,medulla bulbar and cerebellum of the four groups were statistically different(P<0.001),among which group D had the lowest noise and the highest SNR and CNR.There was no significant difference in CT values between the 4 groups(P>0.05).In addition,the subjective scores of the four groups were statistically different,and group D was significantly better than group A,B and C(P<0.05).The subjective score consistency between the two doctors was good,and the kappa value was 0.817(P<0.001).Conclusion Brain Time Stack image fusion technology can effectively reduce image noise and significantly improve image quality.
