摘  要:目的探讨双源CT双能量成像在良、恶性肾脏小肿瘤(Small Renal Masses,SRMs)鉴别诊断中的应用价值。方法收集最大径≤4 cm肾透明细胞癌24例作为恶性组,乏脂性肾血管平滑肌脂肪瘤和肾嗜酸细胞腺瘤10例作为良性组,测量并比较肿瘤、正常肾皮质区在皮质期及髓质期的碘浓度(Iodine Concentration,IC)值和能谱曲线斜率。结果良性SRMs髓质期IC值高于恶性,其差异有统计学意义(P<0.05),良、恶性SRMs皮质期以及正常肾皮质在增强扫描皮质期、髓质期IC值方面差异均无统计学意义(P>0.05)。良性SRMs皮质期、髓质期曲线斜率均大于恶性,皮质期曲线斜率差异有统计学意义(P<0.05),髓质期曲线斜率差异无统计学意义(P>0.05),良、恶性SRMs病变同平面正常肾皮质在增强扫描皮质期、髓质期能谱曲线斜率差异无统计学意义(P>0.05)。结论双源CT双能量成像中髓质期IC值和皮质期能谱曲线斜率可用于良、恶性SRMs的鉴别诊断。Objective To investigate the differential diagnosis value of dual-source CT dual-energy scanning between benign and malignant small renal masses(SRMs).Methods The data of 24 patients with pathologically confirmed renal clear-cell carcinoma with the maximum diameter≤4 cm was included in the malignant group,and the data of 10 patients with fat-poor renal angiomyolipoma and renal oncocytoma was selected as benign group.The iodine concentration and energy spectrum curve of the lesion between the two groups and the normal cortex renis were measured in cortical phase and nephrographic phase.Results Benign SRMs had higher iodine concentration(IC)values than malignant in nephrographic phase,and the difference was statistically significant(P<0.05).There were no statistically significant differences of IC in cortical phase in benign and malignant SRMs and normal renal cortex in cortical phase and nephrographic phase(P>0.05).The slope of benign SRMs in cortical phase and nephrographic phase was higher than that in malignant phase,and there was statistically significant difference in the slope of cortical phase(P<0.05),but there was no significant difference in the slope of the nephrographic phase(P>0.05).There was no significant difference in the slope of energy spectrum curve between benign and malignant SRMs lesions and normal renal cortex in enhanced scan(P>0.05).Conclusion The dual-source CT dual-energy imaging in medullary phase IC value and cortical phase curve slope can be used for the differential diagnosis of benign and malignant SRMs.
