摘  要:目的探讨南京地区妊娠中晚期孕妇补充不同剂量的维生素D与血清维生素D、铁蛋白和血红蛋白水平的相关性。方法选取2019年8月至2020年12月在南京医科大学第一附属医院产检的300例孕妇,于孕24周始分为3组,对照组(未服用维生素D组)100例、400U组(每日服用400 U维生素D组)100例和800U组(每日服用800 U维生素D组)100例,分别于孕24、28、32、36周检测3组孕妇的血清维生素D、铁蛋白及血红蛋白水平。结果对照组孕妇血清维生素D水平随孕周增加逐渐降低[孕24周(41.0±11.8)nmol/L,孕36周(25.2±8.6)nmol/L],400U组及800U组孕妇血清维生素D水平随孕周增加均逐渐增高[孕36周400U组(51.3±6.9)nmol/L、800U组(75.5±11.5)nmol/L],其中,孕36周800U组孕妇血清维生素D水平较400U组提升明显,差异有统计学意义(P<0.05)。此外,对照组孕妇血清铁蛋白及血红蛋白水平随孕周增加逐渐降低,而补充不同剂量维生素D组的孕妇与之相比,至36周时血红蛋白及血清铁蛋白水平均未明显降低[孕36周对照组、400U组及800U组铁蛋白分别为(23.5±5.0)、(25.4±4.9)和(27.5±4.3)μg/L,血红蛋白分别为(117.5±7.3)、(122.0±8.8)、(126.8±6.3)g/L],且800U组孕妇血红蛋白及血清铁蛋白水平明显高于对照组及400U组(P<0.05)。结论南京地区妊娠中晚期孕妇血清维生素D处于低水平状态,妊娠中晚期每日补充800U维生素D才能维持血清维生素D在正常范围内,补充维生素D有利于提升孕妇血清铁蛋白及血红蛋白水平。Objective To investigate the association of different doses of vitamin D intake with serum levels of vitamin D,ferritin and hemoglobin in second and third trimesters of pregnant women in Nanjing.Methods A retrospective study was performed on 300 pregnant women attending antenatal care in the First Affiliated Hospital of Nanjing Medical University from August 2019 to December 2020.They were divided into 3 groups at 24 weeks of pregnancy at random,including control group(no vitamin D,n=100),400U group(400 units of vitamin D daily,n=100)and 800U group(800units of vitamin D daily,n=100).Maternal serum vitamin D concentrations,and ferritin and hemoglobin levels were measured every four weeks starting from 24 weeks of gestation.Results The levels of vitamin D in the control group decreased gradually with the increase of gestation weeks[at 24 weeks of gestation:(41.0±11.8)nmol/L;at 36 weeks of gestation:(25.2±8.6)nmol/L].The serum vitamin D levels in the 400U and 800U groups increased gradually with the weeks of gestation[400U group at 36 weeks of gestation:(51.3±6.9)nmol/L;800U group at 36 weeks of gestation:(75.5±11.5)nmol/L].Furthermore,the serum concentration of vitamin D in the 800U group at 36 weeks of gestation was significantly higher than the 400U group at 36 weeks of gestation(P<0.05).Meanwhile,the serum levels of ferritin and hemoglobin in the control group were reduced gradually with gestation of weeks,while the other two groups had no significant reduction at 36 pregnancy weeks[ferritin in control group,the 400U group and the 800U group:(23.5±5.0),(25.4±4.9)and(27.5±4.3)μg/L;hemoglobin:(117.5±7.3),(122.0±8.8)and(126.8±6.3)g/L].In addition,the levels of hemoglobin and serum ferritin in 800U group were significantly than those in control group and 400U group(P<0.05).Conclusion Maternal serum vitamin D level in the 2rd and 3rd trimester in Nanjing is lower.Daily supplementation of 800U vitamin D is necessary to keep serum vitamin D level in the normal range.Supplementation of vitamin D helps to incr
