经脐单孔腹腔镜卵巢囊肿剥除术治疗卵巢巨大良性囊肿的疗效分析

摘　　要：目的探讨经脐单孔腹腔镜卵巢囊肿剥除术治疗卵巢巨大良性囊肿的疗效和安全性。方法回顾性分析郑州大学第一附属医院2015年1月至2021年12月收治的行腹腔镜卵巢囊肿剥除术的122例卵巢巨大良性囊肿患者的临床资料,其中经脐单孔腹腔镜组患者41例、传统腹腔镜组患者81例,比较观察2组手术时间、术中出血量、术中囊肿破裂率、术后排气时间、术后住院时间、术后抗苗勒氏管激素(AMH)下降率等。结果经脐单孔腹腔镜组手术时间、术中囊肿破裂率、术后AMH下降率分别为(59.113±6.721)min、7.317%、(28.435±12.243)%,均少于传统腹腔镜组的(65.518±7.137)分、28.395%、(41.967±15.719)%,差异均有统计学意义[t=4.961,P<0.001;χ^(2)=7.212,P=0.007;t=3.739,P<0.001]。结论经脐单孔腹腔镜卵巢囊肿剥除术治疗卵巢巨大良性囊肿安全可行,达到了微创与美观的结合。Objective To evaluate the efficacy and safety of transumbilical single site laparoscopic ovarian cystectomy in the treatment of large ovarian benign cyst.Methods The clinical data of 122 patients with large ovarian benign cyst treated with laparoscopic ovarian cystectomy were analyzed retrospectively from January 2015 to December 2021 in the First Affiliated Hospital of Zhengzhou University,included the single site laparoscopy group(n=41)and the traditional laparoscopy group(n=81).The perioperative related indicators of both groups were analyzed and compared,included the operation time,intraoperative blood loss,intraoperative rupture rate of cyst,postoperative exhaust time,postoperative hospital stay and postoperative anti-müllerian hormone(AMH)decline rate.Results The operation time,the intraoperative rupture rate of cyst and postoperative AMH decline rates of the single site laparoscopy group[(59.113±6.721)min,7.317%,(28.435±12.243)%]were less than those of the conventional laparoscopy[(65.518±7.137)min,28.395%,(41.967±15.719)%;t=4.961,P<0.001;χ^(2)=7.212,P=0.007;t=3.739,P<0.001].Conclusion Transumbilical single site laparoscopic ovarian cystectomy is safe and efficacy in the treatment of large ovarian cyst.
