摘　　要：目的评价Nd:YAG泪道激光联合泪道引流管置入治疗鼻泪道阻塞的远期疗效。方法选取2016年3月至2018年3月在江南大学附属医院住院治疗的84例鼻泪道阻塞患者,采用随机数字表法分为观察组和对照组,每组42例。对照组接受常规手术治疗,观察组行Nd:YAG泪道激光联合泪道引流管置入治疗,两组术后均随访36个月。比较两组患者的围术期情况、手术前后的眼表状态(泪液分泌量、泪膜破裂时间)、远期疗效及并发症发生情况。结果观察组的手术时间、术中出血量、术后住院时间均低于对照组,患者治疗满意度高于对照组,差异有显著性(P<0.05)。两组手术前后的泪液分泌量、泪膜破裂时间组内及组间比较差异均无显著性(P>0.05)。术后36个月时,观察组的治疗总有效率为95.2%,明显高于对照组的81.10%,差异有显著性(P<0.05)。观察组术后并发症发生率(19.0%)与对照组(26.2%)比较差异无显著性(P>0.05)。结论Nd:YAG泪道激光联合泪道引流管置入治疗鼻泪道阻塞远期效果满意,并发症少,值得临床应用。Objective To evaluate the long-term effect of Nd:YAG lacrimal duct laser combined with lacrimal duct placement in the treatment of nasolacrimal duct obstruction.Method The 84 patients with nasolacrimal duct obstruction admitted to our hospital from March 2016 to March 2018 were divided into observation group and control group according to a random number table,with 42 cases in each group.The conventional surgical treatment were applied in the control group,while the Nd:YAG lacrimal duct laser combined with lacrimal drainage tube implantation were applied in the observation group.Patients in both groups were followed up for 36 months to observe the long-term efficacy.Result The long-term curative effect of the observation group was significantly higher than that of the control group(P<0.05);there was no major alter in ocular surface state of patients in both groups before and after surgery(P>0.05);the observation group had lower operation time,postoperative hospital stay and amount of blood loss than those in the control group(P<0.05),and the satisfaction with treatment was obviously higher than that in the control group(P<0.05);as for the total number of postoperative complications,the two groups were similar(P<0.05).Conclusion Nd:YAG lacrimal duct laser combined with lacrimal drainage tube implantation in the treatment of nasolacrimal duct obstruction has a satisfactory effect with few complications and has clinical promotion value.
