卵巢中低分化支持-间质细胞瘤的冰冻病理诊断分析

摘　　要：目的探讨卵巢中低分化支持-间质细胞瘤(Sertoli-Leydig cell tumor,SLCT)的术中冰冻病理诊断。方法回顾性分析2007年1月至2022年3月在北京大学人民医院经石蜡病理确诊为卵巢中低分化SLCT的36例住院手术患者的临床表现、术前检查、手术方式、冰冻病理资料。所有患者均具有完整的病史资料及随访信息,术中行快速冰冻病理检查。结果36例患者术前临床诊断为卵巢性索-间质肿瘤7例、卵巢占位性病变17例、低分化癌12例。术中冰冻病理诊断11例考虑为卵巢性索-间质肿瘤,中低分化SLCT可能性大;8例诊断为中低分化的恶性肿瘤;17例诊断为低分化癌。结论SLCT术中冰冻病理诊断正确率偏低,分型未明的冰冻结果可能影响术中治疗决策。详尽的病史、完善的实验室检查及影像学资料、准确的肉眼观察描述、对该病的认识和警惕是提升术中冰冻病理诊断准确率的关键。Objective To explore the diagnostic pitfall of frozen section examination in ovarian low differentiated Sertoli-Leydig cell tumors.Method The clinical manifestations,preoperative examinations,surgical methods,frozen section evaluations and pathologic results from 36 cases of ovarian low differentiated SLCT were collected and analyzed retrospectively.Result 36 cases of ovarian low differentiated SLCT with the following common characteristics:these patients were successively admitted to Peking University People’s Hospital,with complete medical history,follow-up information and frozen section evaluations during operation.Among these cases,7 cases were considered as ovarian sex cord-stromal tumors,17 cases as ovarian mass,and 12 cases as poorly differentiated cancer.Frozen sections results:11 patients were most probably considered of ovarian SLCT,8 patients were diagnosed as low differentiated malignancies,and as many as 17 cell tumors often present diagnostic dilemmas due to their relative infrequency and high degree of morphologic variability.Conclusion The diagnosis of frozen section materials are even challenging.Given that the majority of Sertoli-Leydig cell tumors are unilateral and benign,fertility-sparing surgery can be used as the primary treatment modality.Owing to accurate frozen section evaluations of ovarian lesions,more extensive surgical resections can be avoided.
