摘 要:目的探讨肺癌手术患者围手术期采取免疫营养支持对机体免疫炎症指标、营养状况和肺功能的影响。方法选取2020年1月至2021年10月在郑州市第三人民医院行改良肺癌根治术的70例患者,按照随机数字表法将其均分成观察组(n=35)和对照组(n=35)。对照组采用围手术期常规护理方法,观察组采用围手术期常规护理联合免疫营养支持护理方法,对比2组患者的免疫炎症指标、营养状况和肺功能指标。结果术后3个月,观察组患者血清白介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)水平均低于对照组(t=11.830,P<0.001;t=9.593,P<0.001);血红蛋白水平、白蛋白水平、总蛋白水平、第一秒用力呼气容积、用力肺活量和呼气流量峰值均高于对照组(t=6.109,P<0.001;t=8.251,P<0.001;t=5.574,P<0.001;t=5.896,P<0.001;t=3.580,P<0.001;t=3.906,P<0.001)。结论对于肺癌手术患者,围手术期常规护理联合免疫营养支持可以有效改善患者术后炎症反应和机体营养状况,增强肺功能。Objective To explore the impact of immunonutritional support on immune inflammatory indexes,nutritional status and lung function in patients with lung cancer.Methods Seventy patients undergoing modified radical resection of lung cancer from January 2020 to October 2021 in the Third People’s Hospital of Zhengzhou were selected.They were randomly divided into the control group(n=35)and the observation group(n=35).The patients of the control group were treated with perioperative routine nursing,and the patients of theobservation group were treated with perioperative routine nursing combined with immunonutritional support nursing.The indexes of immune inflammatory,nutritional status and lung function of patients were compared.Results In 3 months after operation,the levels of serum interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the observation group were lower than those in the control group(t=11.830,P<0.001;t=9.593,P<0.001);the levels of hemoglobin,albumin,total protein,forced expiratory volume in one second,forced vital capacity and peak expiratory flow in the observation group were higher than those in the control group(t=6.109,P<0.001;t=8.251,P<0.001;t=5.574,P<0.001;t=5.896,P<0.001;t=3.580,P<0.001;t=3.906,P<0.001).Conclusion For lung cancer patients,routing nursing combined with immunonutritional support in the perioperative period can effectively improve postoperative inflammatory response and nutritional status,and enhance lung function.
