摘 要:目的探讨超声(US)、计算机断层扫描(CT)、磁共振成像(MRI)在背部弹力纤维瘤(ED)中的诊断价值。方法收集2016年9月至2021年9月郑州大学第一附属医院收治的ED患者,均实施肿瘤切除术,经术后病理学确诊为ED。比较US、CT和MRI等3种影像学检查方法的术前诊断准确率和测量精准度。结果纳入的69例患者共94处病变,经US检查的39处肿物中有6处诊断为ED,诊断准确率15.38%;经CT检查的44处肿物中有10处诊断为ED,诊断准确率22.73%;经MRI检查的68处肿物中有34处诊断为ED,诊断准确率50.00%。US、CT和MRI术前诊断准确率总体比较差异有统计学意义(χ^(2)=16.433,P<0.001)。MRI术前诊断准确率高于US、CT(χ^(2)=12.687,P=0.001;χ^(2)=8.331,P=0.004),而US与CT术前诊断准确率比较差异无统计学意义(χ^(2)=0.716,P=0.397)。US、CT、MRI检查测量误差值分别为0.15±0.09、0.11±0.06、0.05±0.03,总体比较差异有统计学意义(F=43.350,P<0.001)。MRI测量误差值低于US、CT(P均<0.001),CT测量误差值低于US(P=0.002)。结论US、CT、MRI都能用于ED术前诊断和肿物直径测量,MRI术前诊断准确率和测量精确度更高。Objective To investigate the diagnostic value of ultrasound(US),computed tomography(CT)and magnetic resonance imaging(MRI)in the elastofibroma dorsi(ED).Methods All the patients with ED were collected from September 2016 to September 2021 in the First Affiliated Hospital of Zhengzhou University.The preoperative diagnostic accuracy and measurement precision of the three imaging examinations(US,CT and MRI)were compared.Results A total of 69 patients with 94 lesions were included.6 out of 39 swellings detected by US were diagnosed as ED,with a diagnostic accuracy of 15.38%;10 out of 44 swellings detected by CT were diagnosed as ED,with a diagnostic accuracy of 22.73%;34 out of 68 swellings detected by MRI were diagnosed as ED,with a diagnostic accuracy of 50.00%.The preoperative diagnostic accuracy of US,CT and MRI was respectively 15.38%,22.73%and 50.00%(χ^(2)=16.433,P<0.001).The preoperative diagnostic accuracy of MRI was higher than those of US and CT(χ^(2)=12.687,P=0.001;χ^(2)=8.331,P=0.004),but the difference between US and CT was not statistically significant(χ^(2)=0.716,P=0.397).The measurement error value of US,CT and MRI was respectively 0.15±0.09,0.11±0.06 and 0.05±0.03(F=43.350,P<0.001).The measurement error value of MRI waslowerthan those of US and CT(P<0.001);the measurement error value of CT was lower than that of US(P=0.002).Conclusion US,CT and MRI can be used for preoperative diagnosis of ED and measurement of the diameter of the mass,with MRI testing being more accurate in preoperative diagnosis and more precise in measurement.
