摘　　要：目的探讨高龄管饲患者血清胃蛋白酶原水平影响因素。方法选择2019年4月至2021年7月住院高龄管饲患者共63例,采用乳胶免疫比浊法测定患者血清胃蛋白酶原(PG)Ⅰ、PGⅡ水平,并计算二者比值PGR(PGⅠ/PGⅡ)。分析不同置管位置、饲管留置时间、饮食状态和慢性萎缩性胃炎(Chronic Atrophic Gastritis,CAG)对其血清胃蛋白酶原(Pepsinogen,PG)水平变化的影响。结果63例高龄管饲患者血清PGⅠ平均水平为(116.11±78.14)ng/ml,PGⅡ平均水平为(19.47±19.88)ng/ml,PGR均值为(6.79±3.19);幽门前、后喂养患者血清PG水平比较,差异无统计学意义(P>0.05);不同饲管留置时间的患者血清PGⅠ、PGⅡ比较,差异有统计学意义(P<0.05);匀浆膳、肠内营养液、清流大米汤和禁食4种不同饮食状态患者血清PG水平比较,差异无统计学意义(P>0.05);慢性萎缩性胃炎高龄管饲患者血清PG水平比较,差异有统计学意义(P<0.05)。结论在一定时间内,随着饲管留置时间的延长,高龄管饲患者血清PGΙ水平逐渐下降,尤其是慢性萎缩性胃炎患者血清PG水平显著降低,应关注长期管饲慢性萎缩性患者胃黏膜的分泌消化功能。Objective To analyze the effects of different tube placement position,retention time,dietary status and CAG on the levels of serum pepsinogen in elderly patients with tube feeding.Methods 63 hospitalized elderly patients with tube feeding from April 2019 to July 2021 were selected.The levels of serum PGⅠand PGⅡwere measured by Latex immunoturbidimetric method,and the ratio of PGⅠto PGⅡwas calculated(PGR).Results Of the 63 elderly tube feeded patients with average serum PGⅠof(116.11±78.14)ng/ml,and average serum PGⅡof(19.47±19.88)ng/ml,and average PGR of(6.79±3.19);There was no significant difference in serum PG levels between pre and post pyloric tube feeding groups(P>0.05);The feeding tube retention time had significant difference in serum PGⅠand PGⅡ(P<0.05);Four different dieatry states were compared among homogenate diet、ENS、rice soup and fasting,the comparison had no significant difference(P>0.05);Comparison of serum PG levels in elderly patients with different chronic gastritis by tube feeding(P>0.05),the difference was not statistically significant.Conclusion The level of serum PG would gradually decreased as the prolonged use of indwelling feeding tube in elderly patients for a set period.The secretion and digestive function of gastric mucosa in patients with long-term feeding tube retained should be concerned.
