摘　　要：目的:评价超声造影与传统示踪剂示踪乳腺癌前哨淋巴结的应用价值。方法:在中国知网、万方数据库、维普数据库、PubMed、Cochrane Library、Web of Science搜索超声造影与传统示踪剂示踪乳腺癌前哨淋巴结的文献,检索时间为建库至2021年07月,2名研究者分别按照纳入和排除标准筛选文献,提取相关数据,交叉核对结果,进行文献质量评价,使用Review Manager 5.3软件进行统计分析。结果:纳入10项研究共925名患者。在患者检出率、准确率方面超声造影与传统示踪剂比较差异无统计学意义(OR=0.93,95%CI:0.46~1.89,P=0.84;OR=1.23,95%CI:0.96~1.57,P=0.10),在阳性检出率方面传统示踪剂效果优于超声造影,差异具有统计学意义(OR=0.18,95%CI:0.06~0.56,P=0.003)。结论:超声造影示踪乳腺癌患者前哨淋巴结阳性检出率方面弱于传统示踪剂,但在患者检出率、准确率方面与传统示踪剂相当,具有良好的应用前景。Objective:To evaluate the application value of contrast-enhanced ultrasound and traditional tracer in tracing sentinel lymph node of breast cancer.Methods:The literatures on sentinel lymph node tracing of breast cancer with contrast-enhanced ultrasound and traditional tracer in CNKI,WanFang,VIP,PubMed,Cochrane Library,Web of Science were researched.The search time was from the establishment of the database to July 2021.Two researchers screened the literatures according to the inclusion and exclusion criteria,extracted relevant data,cross-checked the results,evaluated the quality of the literature,and used the Review Manager 5.3 software for statistical analysis.Results:A total of 10 studies with 925 patients were included.There was no statistically significant difference between contrast-enhanced ultrasound and traditional tracer in the patients detection rate and accuracy rate(OR=0.93,95%CI:0.46~1.89,P=0.84.OR=1.23,95%CI:0.96~1.57,P=0.10).The effect of traditional tracer on the positive detection rate was better than contrast-enhanced ultrasound,and the difference was statistically significant(OR=0.18,95%CI:0.06~0.56,P=0.003).Conclusion:The positive detection rate of sentinel lymph node in breast cancer patients with contrast-enhanced ultrasound is weaker than that of traditional tracer,but the detection rate and accuracy rate for patients are equivalent to traditional tracer,and it has good prospects.
