摘 要:目的:分析单平面和双平面超声介导下前列腺穿刺活检的检出率及安全性。方法:回顾性分析2020年2月-2021年1月在江苏省常州市中医医院泌尿外科行前列腺穿刺活检的155例患者的临床资料,将其中采用单平面超声介导前列腺穿刺活检的78例患者设为单平面组;采用双平面超声介导前列腺穿刺活检77例患者设为双平面组。比较两组患者的疾病检出率以及并发症的发生情况,并观察分析其超声图像。结果:穿刺活检结果显示,单平面组,经病理结果确诊为前列腺癌患者43例(55.13%),前列腺增生患者30例(38.46%),前列腺炎患者5例(6.41%);双平面组,经病理结果确诊为前列腺癌患者43例(50.65%),前列腺增生患者30例(44.16%),前列腺炎患者5例(5.19%)。超声图像结果显示,单平面超声在78例患者中,发现癌结节33例,增生结节19例;双平面超声在77例患者中,发现癌结节32例,增生结节24例。两组PCA以及前列腺增生检出率比较,差异无统计学意义(P>0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:单平面和双平面超声介导下前列腺穿刺活检均是安全有效的方法,但双平面超声介导在检测过程中更为便捷。Objective:To analyze the detection rate and safety of prostate puncture biopsy mediated by uniplane and biplaneultrasound.Methods:The clinical data of 155 patients who underwent prostate biopsy in Changzhou Hospital of TraditionalChinese Medicine from February 2020 to January 2021 were retrospectively analyzed.Among them,78 patients who underwentuniplane ultrasound-mediated prostate puncture biopsy were set as the single-plane group;77 patients who underwent biplaneultrasound-mediated prostate puncture biopsy were set as biplane group.The disease detection rate and the occurrence ofcomplications of the two groups of patients were compared,and the results of ultrasound images were observed and analyzed.Results:The puncture biopsy results showed that in the single-plane group,43 cases(55.13%) were diagnosed as prostate cancerby pathological results,30 cases(38.46%) as prostate hyperplasia,and 5 cases(6.41%) as prostatitis.In the biplane group,43cases(50.65%) were diagnosed as prostate cancer by pathological results,30 cases(44.16%) as prostatic hyperplasia,and 5 cases(5.19%) as prostatitis.Ultrasound image results showed that in 78 patients of single-plane ultrasound,33 cases had cancerousnodules and 19 cases had hyperplastic nodules;in 77 patients of biplane ultrasound,32 cases had cancerous nodules and 24 caseshad hyperplastic nodules.There was no statistically significant difference in the detection rate of PCA and prostate hyperplasiabetween the two groups(P>0.05).There was no statistically significant difference in the incidence of complications between thetwo groups(P>0.05).Conclusion:Both uniplane and biplane ultrasound-mediated prostate puncture biopsy are safe and effectivemethods,but biplane ultrasound-mediated biopsy is more convenient in the detection process.
