超声骨刀技术联合医用胶原蛋白海绵在颌骨囊肿中的应用

摘　　要：目的:探讨超声骨刀技术联合医用胶原蛋白海绵在颌骨囊肿患者中的治疗效果。方法:选择2018年4月~2021年3月颌骨囊肿患者100例,随机数字表法分为两组,各50例。对照组采用超声骨刀技术治疗,观察组采用超声骨刀技术联合医用胶原蛋白海绵治疗。记录术前及术后7 d两组患者临床资料,并随访12个月。比较两组围术期指标、术后疼痛情况、并发症。结果:两组术后7 d VAS评分明显降低,观察组术后7 d VAS评分低于对照组(P<0.05)。观察组术中出血量低于对照组(P<0.05)。两组术后7 d均伴有炎性因子水平升高,而观察组术后7 d IL-1α、IL-4、IL-6、TNF-α水平低于对照组(P<0.05);两组随访期间并发症发生率无统计差异(P>0.05)。结论:超声骨刀技术联合医用胶原蛋白海绵用于颌骨囊肿患者中创伤较小,能减轻术后疼痛,降低炎症反应,未增加术后并发症发生率,值得推广应用。Objective:To investigate the therapeutic effect of ultrasonic bone knife technique combined with medical collagen sponge in patients with jaw cyst.Methods:A total of 100 patients with jaw cysts from April 2018 to March 2021 were selected,and randomly divided into two groups with 50 cases in each group.The control group was treated with ultrasonic osteotome,and the observation group was treated with ultrasonic osteotome combined with medical collagen sponge.The clinical data of the two groups were recorded before operation and 7 days after operation,and were followed up for 12 months.The perioperative indicators,postoperative pain and complications were compared between the two groups.Results:The pain scale indicated that VAS scores of both groups decreased significantly 7 days after operation.VAS score of observation group was lower than that of control group at 7 days after operation(P<0.05).The amount of intraoperative blood loss in observation group was lower than that in control group(P<0.05).Inflammatory factors increased in both groups after 7 days of operation.The levels of IL-1α,IL-4,IL-6 and TNF-αin the observation group were lower than those in the control group(P<0.05).There was no statistical difference in the incidence of complications between the two groups during treatment(P>0.05).Conclusion:Ultrasound bone knife technology combined with medical collagen sponge can reduce postoperative pain in patients with jaw cyst with less trauma,and the treatment method has little effect on inflammatory factors and does not increase the incidence of postoperative complications,which is worthy of popularization and application.
