摘　　要：目的:分析颅内肿瘤患者肿瘤切除术前凝血参数的差异。方法:选取我院2018年01月至2018年12月行颅内肿瘤切除术患者602例,回顾分析各组患者术前的凝血参数:活化部分凝血活酶时间(activated partial thromboplastin time,APTT)、凝血酶原时间(prothrombin time,PT)、纤维蛋白原(fibrinogen,FIB)含量、血小板计数(platelet count,PLT),分组进行组间比较并与57例无肿瘤患者(微血管减压术患者)进行比较。结果:颅内肿瘤患者APTT值低于无肿瘤患者,而FIB高于无肿瘤患者。采用LSD法进行颅内肿瘤患者组间比较:Ⅳ级胶质瘤组APTT值低于垂体瘤组(P=0.03)与颅咽管瘤组(P=0.00);转移瘤组APTT值低于颅咽管瘤组(P=0.02);脑膜瘤组、Ⅳ级胶质瘤组、转移瘤组PT值均低于颅咽管瘤组(P均<0.05)。结论:颅内肿瘤患者术前存在不同程度的凝血功能亢进,且具有差异性,应引起临床重视。Objective:To analyze the coagulation parameters of patients with intracranial tumors before tumor resection.Methods:602 patients who underwent intracranial tumor resection in our hospital from January 2018 to December 2018 were selected.The coagulation parameters before tumor resection of each group were retrospectively analyzed,including activated partial thromboplastin time(APTT),prothrombin time(PT),fibrinogen(FIB)content,platelet count(PLT),grouped for comparison between groups and compared with 57 patients without tumors(microvascular decompression patients).Results:The APTT values of patients with intracranial tumors in each group were lower than those without tumors,while FIB was higher than those without tumors.LSD method was used to compare intracranial tumor patients:APTT of gradeⅣglioma group was lower than that of pituitary tumor group(P=0.03)and craniopharyngioma group(P=0.00).APTT of metastasis group was lower than craniopharyngeal tube tumor group(P=0.02).PT values of meningioma group,gradeⅣglioma group,and metastatic tumor group were lower than those of craniopharyngioma group,with P<0.05.Conclusion:Patients with intracranial tumors have different degrees of hypercoagulability before surgery,and there are differences,which should attract clinical attention.
